answer text <p>Due to the way in which data is collected on incidents by the Human Fertilisation and Embryology Authority (HFEA) and the National Reporting and Learning Systems (NRLS), it is not possible to make a direct comparison as both sets of data are recorded by separate bodies and the incidents are not recorded in the same way.</p><p>The HFEA monitors all reported incidents in order to ensure that clinics learn from them and prevent re-occurrence. Overall, the number of incidents has remained at 0.6% per treatment cycle for the last few years. There are about 80,000 treatment cycles per year and there have been five grade A incidents, the most serious, since 2016.</p><p>NHS England and NHS Improvement operate the NRLS, which collects reports of patient safety incidents from across the National Health Service. Patient safety incidents can be reported through a variety of routes by individual NHS staff, including through local trust risk management systems and web-based e-forms, including an open access e-form. Incidents are reported voluntarily.</p><p> </p>

answer text <p>The decision to adopt a majority surface route in the Wendover area was supported by the House of Commons Select Committee during the passage of the High Speed Rail (London to West Midlands) Act 2017, who were fully presented with the costs and benefits of the presented options.</p><p>The alternative Wendover Mined Tunnel Proposal was subsequently assessed by HS2 Ltd and by independent consultants as being harder and more expensive to construct than the consented scheme. This assessment was provided to the constituency MP and local councillors at the time, and again in February 2020.</p><p>The Department does not release information provided in confidence by external suppliers or consultants.</p><p> </p> more like this

answer text <p>On 22 October 2020 the Home Office published the Hong Kong British National (Overseas) Visa Impact Assessment, which set out estimated volumes of those who would take up the British National (Overseas) visa as well as indicative economic impacts. This estimated the route could have a net positive impact on the UK, benefiting the economy by between £2.4 and £2.9 billion over five years. It is available on Gov.uk</p><p>On 31 January 2022, the Home Office published data on the characteristics of British National (Overseas) visa holders, based on information submitted on their visa applications and the results from a sample survey. answer text <p>The recently published sixth national otter survey of Wales (2015-18) showed a decline in the percentage of survey sites which had evidence of otters for the first time since the surveys began in the late 1970s. In England, the Environment Agency, Natural England and the Mammal Society are collaborating on a new national otter survey of England which is due to start later this year. By the end of next year we hope to have sufficient data to understand whether the England population has continued to recover and what threats, if any, may hinder recovery locally or nationally.</p><p> </p><p>Under the Environment Act, this government has set a world leading target to halt the decline in species by 2030, and will also be setting targets to restore water quality. We expect actions to deliver these targets to support the continued recovery of native species like the otter.</p><p> </p><p>My department will publish a Green Paper early this year which will look at how the regulatory framework can help drive the delivery of our 2030 target and reverse declines of species.</p><p> </p><p>We are creating a Nature Recovery Network which will be a bigger, better quality and increasingly connected network of places that are richer in wildlife, support the recovery of species, and are more resilient to climate change and other pressures. At the core of the Network will be our existing best areas for nature, including protected sites and National Nature Reserves. As committed to in the 25 Year Environment Plan, we will also create or restore 500,000 hectares of additional wildlife-rich habitat beyond these existing areas. These actions will also help recover species like the otter.</p><p> </p>
