answer text <p>Hepatic encephalopathy is an important end stage complication of liver disease. Monitoring at a national level is difficult because there is no specific code in the 10th edition of the International Classification of Diseases system (ICD10). The ICD10 guide recommends that patients with hepatic encephalopathy are classified as K72.9; hepatic failure, un-specified. Public Health England is working on raising the profile of hepatic encephalopathy with the Lancet Commission.</p><p> </p> more like this

answer text <p>Whilst NHS England is responsible for the overall national approach to improving clinical outcomes for people with liver disease, Public Health England (PHE) is working with the Lancet Commission on Liver Disease. This work involves regular meetings with Professor Richard Williams, (Lead, Lancet Standing Commission into Liver Disease in the UK), and attending action planning meetings.</p><p> </p><p> </p><p> </p><p>PHE is working with NHS England and liver charities to respond to the key recommendations of the Lancet Commission and will produce a Liver Disease Framework in summer 2015 outlining the scope of its activities.</p><p> </p><p> </p><p> </p><p>This will cover PHEs extensive programme of activities to tackle the three main risk factors for liver disease: alcohol, obesity and Hepatitis B &amp; C; work with the Lancet Commissioners and GPs to strengthen detection and interventions for early stage liver disease and for identification of high risk patients; support a national needs assessment for liver disease services in the NHS.</p><p> </p>

However, liver cancer outcomes for the specialised services which NHS England commissions are currently audited and a national audit of outcome dimensions for the treatment of hepatitis C will be established by NHS England in due course. In addition, PHE is working with the Lancet Commission on Liver Disease to use routine data to investigate elements of care for people with advanced liver disease.</p><p> </p><p> </p><p> </p><p>We are also taking action to prevent people developing liver disease in the first place by tackling two of the main causes of liver disease – obesity and alcohol misuse. This includes:</p><p> </p><p> </p><p> </p><p>- our Call to Action on Obesity, which sets out two national ambitions for a downward trend in level of excess weight in children and adults by 2020; and</p><p> </p><p> </p><p> </p><p>- our Alcohol Strategy, which aims to cut the number of people drinking above the lower-risk guidelines.</p><p> </p>

In addition, PHE is working with stakeholders to develop a liver disease framework.</p><p> </p><p> </p><p> </p><p>We are also taking action to prevent people developing liver disease in the first place by tackling two of the main causes of liver disease – obesity and alcohol misuse. This includes:</p><p> </p><p> </p><p> </p><p>- our Call to Action on Obesity, which sets out two national ambitions for a downward trend in level of excess weight in children and adults by 2020;</p><p> </p><p>- our Alcohol Strategy, which aims to cut the number of people drinking above the lower-risk guidelines; and</p><p> </p><p>- PHE’s five outcome focussed priorities which includes helping people to live longer and more healthy lives by reducing preventable deaths and the burden of ill health associated with, amongst others, alcohol.</p><p> </p><p><strong> </strong></p><p> </p><p>In addition, the National Institute for Health and Care Excellence is developing clinical guidance on the assessment and management of cirrhosis. The guidance is due to be published in May 2016.</p><p> </p><p><strong> </strong></p><p> </p>

answer text <p>Public Health England (PHE) has a comprehensive range of action aimed at reducing the incidence and mortality from liver disease. It monitors the incidence, mortality and outcomes of treatment for liver disease and the risk factors: alcohol obesity and hepatitis B and C. PHE has a wide range of action to tackle unhealthy alcohol consumption, obesity and viral hepatitis through strengthening local action, promoting healthy choices, and giving appropriate information to support healthier lives.</p><p> </p><p> </p><p> </p><p>In response to the All Party Parliamentary Hepatology Group report PHE has committed to producing a PHE Liver Disease Framework. This will focus on public health action to tackle risk factors for liver disease and inequalities in relation to liver disease. Work has already begun to bring together expertise within PHE on the major risk factors for liver disease (alcohol, hepatitis B and C and obesity), data on liver disease and its risk factors and on death and dying from liver disease. PHE has also issued liver profiles to each local authority area, which include information about hepatitis C, as well as modelling tools to assist local commissioners in establishing need at local level. Many of the actions to tackle the major risk factors require a coordinated approach between PHE and NHS England.</p><p> </p><p> </p><p> </p><p>NHS England is responsible for delivering improvements in outcomes against the NHS Mandate and in line with the NHS Outcomes Framework. NHS England is adopting a broad strategy for delivering improvements in relation to premature mortality, working with commissioners and PHE to support clinical commissioning groups in understanding where local challenges lie and in identifying the evidence in relation to the priorities for reducing mortality at a national level.</p><p> </p>

answer text <p>Public Health England plans to publish its framework for tackling liver disease in the summer of 2015.</p><p> </p><p> </p><p> </p><p>The framework will lay out clearly the scope of Public Health England activities. This will range from the investigation of hepatitis viruses to support local authorities in their work to tackle the effects of alcohol consumption and obesity on liver disease. It will also cover the life course from antenatal screening, vaccination of babies born, to mothers with chronic Hepatitis B, to improving the care of patients dying from liver disease.</p><p> </p><p> </p><p> </p><p>Public Health England is not intending to hold a formal consultation on the framework but is conducting extensive engagement with a wide range of stakeholders in its development. These include NHS England, the liver disease related charities, hepatologists (liver disease specialists) from across the country and local authority representatives.</p><p> </p> more like this

At each stage officials at the Department are in discussion with PHE.</p><p> </p><p> </p><p> </p><p>In response to the All Party Parliamentary Hepatology Group report PHE has committed to producing a Liver Disease Framework. This will focus on public health action to tackle risk factors for liver disease and inequalities in relation to liver disease. Work has already begun to bring together expertise within PHE on the major risk factors for liver disease (alcohol, hepatitis B and C and obesity), data on liver disease and its risk factors and on death and dying from liver disease. Many of the actions to tackle the major risk factors require a coordinated approach between PHE and NHS England.</p><p> </p><p> </p><p> </p><p>A meeting will be held on 25 November between PHE officials, NHS England officials, Departmental officials and representatives of Charities to outline the emerging framework and to seek comment and advice. The Lead for Liver Disease at PHE is working closely with the National Clinical Director for Gastroenterology. The Department is kept informed of PHE’s work programme on liver disease through discussion and communication.</p><p> </p><p> </p><p> </p>
