answer text <p>The panel was developed to take themes and systemic issues that come out of various case reviews and make recommendations for improvements. The Department understands the importance of the work of the Serious Case Panel and is committed to getting this right. We need to do so carefully and therefore we are iterating where necessary to ensure it is effective. The Secretary of State will update the House in due course.</p> more like this
