answer text <p>The Human Fertilisation and Embryology Authority (HFEA) has advised that, with regard to the Human Fertility journal article, it has nothing to add to the answers given to the noble Lord on 22 October 2008 (Official Report WA110), 9 July 2012 (Official Report, cols. WA 202-3) and 8 January 2013 (WA23). Data regarding the cohort of patients described in Fertility and Sterility (volume 101, issue 4, pages 967–973) were not shared with the HFEA. However, the HFEA has access to the published article.</p><p> </p><p> </p><p> </p><p>It would be for clinicians and professional bodies to decide on the evidence required to predict the cohort of patients that may develop ovarian hyperstimulation syndrome and to treat symptoms in accordance with guidelines.</p><p> </p><p> </p><p> </p><p>As outlined in the answer given on 23 Jan 2013 (Official Report WA217), the HFEA would refer to evidence based guidelines, produced by the National Institute for Health and Care Excellence, on these matters.</p><p> </p>

answer text <p>Further to answers given on 12 September (HC Deb, col 750w) the Human Fertilisation and Embryology Authority (HFEA) has advised that if mitochondrial donation was permitted in treatment it is not anticipated that this would impact on the incidence of ovarian hyper stimulation syndrome (OHSS). This is because any egg donation or collection processes necessary for mitochondrial donation would be carried out at HFEA licensed fertility clinics in line with the guidance outlined in the HFEA Code of Practice. Mitochondrial donation raises no new issues in respect of OHSS as the women who may donate eggs will be subject to the same protocols as those donating to fertility treatment.</p><p> </p> more like this

answer text <p>The Government has consistently stated that abortion on the grounds of gender alone is illegal and repeated this in new guidance entitled <em>Guidance in Relation to Requirements of the Abortion Act 1967 - for all those responsible for commissioning, providing and managing services provision</em> which was published on 23 May this year. answer text <p>Free National Health Service funded sight tests are available to eligible groups with the purpose of detecting signs of injury, disease or abnormality and giving an individual a written prescription for glasses or referring them to a doctor for further examination. Funding is also available under the NHS voucher scheme for the provision of spectacles with a prescription power to correct a defect of sight.</p><p> </p><p> </p><p> </p><p>Clinical commissioning groups are responsible for commissioning services to meet the needs of their local population. This could include services over and above the NHS sight test, such as behavioural optometry. answer text <p>The Health and Social Care Information Centre have provided the following data concerning patients with a secondary diagnosis of dementia by clinical commissioning group (CCG) of residence for individuals aged over 65 and over 75, for 2013-14 (provisional):</p><p> </p><p> </p><p> </p><p>(a) the rates of admission to hospital per age group population;</p><p> </p><p>(b) the total hospital bed-days per age group population; and</p><p> </p><p>(c) the rates of admission to hospital from a nursing home or residential care home per age group population.</p><p> </p><p> </p><p> </p><p>Please be aware that data for 2013-14 is provisional and is therefore subject to change.</p><p> </p><p> </p><p> </p><p>It should be noted that some patients who live in nursing homes or residential care may have their source of admission recorded as 'usual place of residence' by the hospital staff but we cannot identify these.</p><p> </p><p> </p><p> </p><p>The table is attached.</p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p>

answer text <p>We are advised by NHS England that the Improving Quality in Physiological Services programme, run jointly by the United Kingdom of Accreditation Service and the Royal College of Physicians, is raising the profile of accreditation and quality assurance schemes for physiological diagnostic services. answer text <p>The provision of fracture liaison services (FLS) is a matter for local clinical commissioning groups (CCGs). Information concerning the number of hospitals in England which presently have a linked FLS is not collected centrally.</p><p> </p><p> </p><p> </p><p>NHS England supports the FLS model recommended by the International Osteoporosis Foundation and the National Osteoporosis society and recognises this model as best practice. NHS England is working with CCGs to support them to develop appropriate local services and services are steadily improving.</p><p> </p> more like this

answer text <p>The attached tables list the number of incidents described as prescribing errors reported to the National Reporting and Learning System (NRLS) by month:</p><p> </p><p> </p><p> </p><p>Table 1: Prescribing errors reported as occurring in acute/general hospitals in England April 2010 to March 2014</p><p> </p><p>Table 2: Prescribing errors reported as occurring in general practitioner (GP) surgeries in England, April 2010 to March 2014</p><p> </p><p>Table 3: Prescribing errors reported as occurring in community pharmacies, April 2010 to March 2014</p><p> </p><p>Table 4: Prescribing errors reported as occurring in nursing homes in England, April 2010 to March 2014</p><p> </p><p> </p><p> </p><p>These are counts of reported incidents (any unintended or unexpected incident which could have, or did, lead to harm for one or more patients receiving National Health Service-funded healthcare) and do not represent the number of incidents which actually occurred. Whilst acute hospitals routinely uploaded reported incidents to the NRLS, routine reporting from GP surgeries, care homes, and community pharmacies is less widely established.</p><p> </p><p> </p><p> </p><p>We do not routinely collect data on all prescribing errors; however, the General Medical Council has undertaken discrete studies to assess the prevalence of prescribing error. These found a mean error rate of 8.9 errors per 100 medication orders for NHS acute trusts. Prescribing or monitoring errors in General Practice were detected for one in eight patients, involving around one in 20 of all prescription items.</p><p> </p><p> </p><p> </p><p> </p><p> </p>
