2015/2016 incident management records show the Department captured 4 data breaches, 1 of which were sufficiently serious to report to the ICO. This was a loss by a 3rd party supplier working on behalf of the Office of Civil Society (OCS) collecting survey data. In 2017 we introduced a more robust method of incident reporting. In 2016/17 the Department captured 8 data breaches of which 3 were reported to the ICO.
