Officials from the Department have not had any recent discussions with the Care Quality Commission on integrating line flushing policies and practices into the assessment framework for care provided in England.</p><p> </p><p>General guidance on prescribing and the use of medicines is published by the National Institute for Health and Care Excellence in an online-only format. Good clinical practice is to flush an intravenous line with saline after a medicine has been administered, to ensure the full dose is delivered to the patient. answer text <p>The Medicines and Healthcare products Regulatory Agency (MHRA), the Government agency responsible for ensuring that medicines and medical devices work and are acceptably safe, has not made an assessment on the case for reviewing the expiry dates of antibiotics.</p><p>Medicine expiry dates, including for antibiotics, are necessary to ensure that the safety and effectiveness of a medicine is maintained over its long-term shelf life. The active ingredient in many medicines can degrade over time resulting in a loss of potency or the formation of impurities in the product. Physical changes to a medicine such as discolouration, may also occur upon prolonged storage. Medicine expiry dates are supported by stability studies completed by the pharmaceutical company, which demonstrate that a medicine remains safe and effective throughout its shelf life. Any change to the expiry date of a medicine requires an independent review of the stability data by the MHRA.</p><p>Companies can and often do extend the shelf life of their medicines once the product is on the market, and as additional stability data become available. It is not possible, however, to extend the expiry date of all medicines unilaterally in the absence of supporting stability data.</p>

answer text <p>The National Institute for Health and Care Excellence (NICE) is the independent body responsible for developing evidence-based guidance for the National Health Service on whether new medicines represent a clinically and cost-effective use of resources. NICE published guidance in August 2022 recommending both ceftazidime with avibactam and cefiderocol for treating severe drug-resistant gram-negative bacterial infections. This was as part of a project to test a new health technology evaluation process and payment model for antimicrobial products. NHS England has now issued contracts with the suppliers and cefiderocol and ceftazidime with avibactamare available for clinicians to prescribe in line with NICE’s recommendations.</p> more like this

answer text <p>The UK Health Security Agency (UKHSA) English surveillance programme for antimicrobial utilisation and resistance report and programme aim to develop and maintain robust surveillance systems for monitoring and reporting trends in antimicrobial use and resistance.</p><p>Alongside surveillance of antibiotic consumptions trends, the report presents evidence pooled from a literature scoping review investigating the association between elements of health inequalities and risk of antimicrobial prescribing in High Income Countries. Trends were identified for sex, with females having the highest overall antibiotic use except for indications including acne, respiratory tract infections and pharyngitis, amongst others.</p><p>The report also presents data on the rate of COVID-19 therapeutics across the country by sex and age. Antibiotic prescribing surveillance data are shared with NHS England and at cross-Government boards on a quarterly basis, as well as being published on the Fingertips tool. UKHSA are committed to identifying surveillance reporting gaps to understand the impact of antimicrobial prescribing and resistance by sex, and on Core20PLUS populations.</p>

answer text <p>No assessment has been made. Since 2019, the Government has restricted the export and hoarding of some medicines already placed on the United Kingdom market where there is evidence of a critical shortage, or a risk of a critical shortage, which could adversely impact UK patients.</p><p>For unbranded generic medicines the Department relies on competition to keep prices down. This has led to some of the lowest prices in Europe and allows prices to react to the market. In an international market this ensures that when demand is high and supply is low, prices in the UK can increase to help secure the availability of medicines for UK patients. Branded medicines spend is controlled through the Branded Health Service Medicines (Costs) Regulations 2018 and the 2019 voluntary scheme for branded medicines pricing and access.</p><p>In the UK, anti-competitive behaviour, including hoarding, is a matter for the Competition and Markets Authority (CMA). The Department asks the CMA to investigate where it has concerns about abuses of competition law for generic medicines and where companies are found by the CMA to be in breach of the legislation, the Department will seek damages and invest that money back into the National Health Service.</p>

answer text <p>The Government relies on competition to drive down prices of generic medicines such as antibiotics. This has led to some of the lowest prices in Europe. Freedom of pricing enables supplies to put prices up quickly to ensure we maintain continuity of supply despite increasing prices within the global market. In secondary care, tendering for antibiotics on a frequent basis ensures best value and supply resilience is optimised.</p><p>No supplier should use the current situation as an opportunity to exploit the National Health Service. Where companies are found to be abusing their dominant position by charging excessive and unfair prices, the Competition and Markets Authority (CMA) can take action against businesses and individuals engaged in anti-competitive conduct. The CMA is working to establish the facts of what is currently happening in the antibiotic market. They stand ready to take action if there is evidence of anti-competitive behaviour that breaks the law.</p> more like this

answer text <p>Public health issues are being actively considered as part of the Government’s trade policy development. The Government has been clear that it will uphold the United Kingdom’s high levels of public, animal, and plant health.</p><p>The UK’s five-year national action plan for antimicrobial resistance includes ambitions to reduce antimicrobial use in human and animal medicine. Public Health England (PHE) works with the National Health Service to develop and maintain robust surveillance systems for monitoring and reporting trends in antimicrobial use.
