We are unsure what is meant by Health and Care Transformation Boards. However in regards to Sustainability and Transformation Plans (STPs):<\/p>

STP footprints are not statutory bodies, but collective discussion forums which aim to bring together health and care leaders to support the delivery of improved health and care based on the needs of local populations. They do not replace existing local bodies, or change local accountabilities. Each footprint has been asked to determine governance arrangements for agreeing and implementing their STP. Individuals who are leading the development of STPs within each footprint, which include National Health Service provider Chief Executives, Clinical Commissioning Group Accountable Officers, local authority senior leaders, are responsible for convening and chairing system-wide meetings, facilitating the open and honest conversations that will be necessary to secure sign up to a shared vision and plan. In the overwhelming majority of cases this is a voluntary, non-statutory role and they are not being paid over and above the remuneration they receive for their existing role. The indicator within the Public Health Outcomes Framework brings together the range of information on levels of preventable sight loss in a single place.<\/p>
The indicator is made up of four sub-indicators which are measuring the crude rates of Age-Related Macular Degeneration, glaucoma and diabetic retinopathy, and of the rate of sight loss certifications per 100,000 population.<\/p>
The latest data published by the Public Health Outcomes framework show that there were the following new certifications of visual impairment in 2013/14:<\/p>
<\/p>
- 11,055 for age related macular degeneration, a decrease of 90 from 2012/13;<\/p>
- 3,432 for glaucoma, an increase of 141 from 2012/13;<\/p>
- 1,563 for diabetic eye disease, a decrease of 29 from 2012/13; and<\/p>
- 22,911 overall new certifications (all causes), an increase of 264 from 2012/13.<\/p>
<\/p>
Early detection is essential to tackling preventable sight loss.
