Since the publication of Hard Truths: The Journey to Putting Patients First, the Government has taken huge strides to boost safety, transparency, compassion and accountability in the National Health Service. We have legislated to make the Care Quality Commission independent, and a new rigorous inspection regime for hospitals, primary care and adult social care has been introduced, resulting in a clear rating on the quality of care. We have also introduced a robust special measures regime for struggling hospitals, so that poor care is identified and rectified. In addition, we have legislated to place a new duty of candour and fit and proper person requirement for directors of NHS bodies, as well as to create a new offence of wilful neglect and ill treatment. This Government has also placed an unprecedented amount of data on the MyNHS website, so that the public can see how their local services perform.<\/p>

There have been a series of recent checks to confirm the adequacy of the IT systems used in the national security vetting process. These include: a comprehensive Defence Information Assurance audit, penetration tests and the re-awarding of the vetting system by the Defence Assurance Information Secretary. The Prime Minister set an ambitious challenge to the National Health Service to ensure that a minimum of 24% of patients who required radical radiotherapy were able to access inverse planned Intensity Modulated Radiotherapy (IMRT) from April 2013. This ambition was achieved in May 2014. The success of this planned expansion of access is demonstrated in the increase in the number IMRT episodes, which have risen from just over 8,500 per year in 2012-13 to a projected figure of over 25,000 for 2014-15.<\/p>

NHS England launched a 12 week consultation on proposed changes to the shape of Stereotactic Radiosurgery and Stereotactic Radiotherapy services across the country on 3 November 2014, which will run until 26 January 2015. Following this consultation, and once a decision has been made by NHS England about the future configuration of services; NHS England will launch a procurement exercise to ensure the right level of service is accessible to patients, regardless of where they live.<\/p>

Patient experience surveys are a valuable source of evidence and the results are used in a range of ways, including the assessment of National Health Service performance as well as in regulatory activities such as registration, monitoring ongoing compliance and reviews. The Care Quality Commission has developed a new Intelligent Monitoring tool to give inspectors a clear picture of the areas of care that need to be followed up within an NHS acute trust or a specialist NHS trust. The system is built on a set of indicators that look at a range of information including patient experience, staff experience and performance.<\/p>

In addition to the patient experience surveys hospital boards and other providers and commissioners of services can also consider the results of the Friends and Family Test (FFT) to consider the implications for quality and safety. While not a traditional survey, the FFT provides near real-time feedback to identify both good and poor quality patient experience. A NHS England review of the FFT found that it is performing well as a service improvement tool, with 85% of trusts reporting that it is being used to improve patient experience, and 78% saying that FFT has increased the emphasis placed on patient experience in their trusts.<\/p>

HES figures are available from 1989-90 onwards. Changes to the figures over time need to be interpreted in the context of improvements in data quality and coverage (particularly in earlier years), improvements in coverage of independent sector activity (particularly from 2006-07) and changes in NHS practice. For example, apparent reductions in activity may be due to a number of procedures which may now be undertaken in outpatient settings and so no longer include in admitted patient HES data.<\/p>
