Doctors wishing to enter specialty training in the United Kingdom need to have completed an undergraduate medical degree and two years of foundation training, after which they can enter a six-year specialty training programme. This comprises of one of three two-year core training programmes (Core Medical Training, Acute Care Common Stem in Acute Medicine or Level 1 Paediatric training), followed by four years of dermatology training, and upon successful completion they can gain registration with the GMC as a specialist doctor.<\/p>

The number of medical specialty training places that are available each year is set by Health Education England (HEE) and based on their assessment of service gaps and predicted workforce needs.<\/p>

The Department has not made an estimate of the number of adults with atopic dermatitis in Strangford or in England. Health is a devolved subject; therefore it would be for the devolved administrations to comment on atopic dermatitis in Scotland, Wales and Northern Ireland. The National Institute for Health and Care Excellence (NICE) estimates that atopic eczema (atopic dermatitis) is common skin condition that affects between 15\u201320% of school-age children and 2\u201310% of adults.<\/p>

<\/p>

Clinical commissioning groups are responsible for commissioning the majority of dermatology services and are best placed to understand the needs of their local population. Most patients - adults and children - with skin problems can be well managed by primary care with support from dermatology departments in the community or local hospitals. The management and treatment of atopic eczema is well established.<\/p>

<\/p>

To support local commissioners and clinicians to shape and manage high quality services, in 2007 NICE produced the guideline Atopic eczema in under 12s: diagnosis and management,<\/p>

which sets out best practice in the diagnosis and management of the condition. Diagnosis and treatment at an early stage will of course benefit children as they reach adulthood. In order to understand the effectiveness of atopic eczema treatment and services, NICE recommends the use of validated patient experience tools including the Dermatology Life Quality Index. This assesses a patient\u2019s quality of life and response to treatment.<\/p>

For those patients with the most severe forms of atopic eczema, who cannot be managed through routine access to treatments provided through primary or secondary care, a referral to a specialised dermatology service may be appropriate. NHS England commissions services for people with rare and complex skin conditions and has set out what providers must have in place in order to offer specialist dermatology care, supporting equity of access for patients wherever they live. These services may provide more intensive therapies with the involvement of a range of health and care professionals, subject to a patient\u2019s needs. There is a range of information to raise awareness of the signs and symptoms of skin conditions such as atopic eczema on the NHS Choices website. In addition to this, in June 2014 Public Health England (PHE), in conjunction with the Department and NHS England, ran a Be Clear on Cancer campaign in the South West of England to raise awareness of the signs of skin cancer including information on how to reduce the risks of getting skin cancer. Health Education East Midlands facilitated a meeting on 15 June 2015 of dermatology consultants and nurses from Nottingham University Hospital and Circle Nottingham. This was a productive meeting and considered the best approach to working arrangements which would provide a good quality service for patients and training opportunities for supporting and developing consultants.<\/p>

The original contract for the Nottingham Treatment Centre was signed in July 2005 between the Secretary of State for Health, the local primary care trusts (PCTs) and Circle after approval of a business case for the scheme by the Department. The services provided by the Treatment Centre include dermatology. The Treatment Centre became operational in July 2008 after construction completion.<\/p>

The Department was not involved directly or indirectly in the reprocurement of the services. On 1 April 2013 the Department\u2019s responsibility under the contract passed to NHS England. The reprocurement was managed locally by the clinical commissioning group and the local area team of NHS England; the contract was awarded to Circle after a competitive procurement process.<\/p>

Since 1 April 2013, NHS England has published information factsheets for a range of purposes. The main factsheet resource for commissioners has been produced to provide the costs and benefits of 25 specific interventions to reduce premature deaths:<\/p>

Although there are currently no plans to produce a commissioning factsheet for dermatology, National Institute for Health and Care Excellence guidance in areas such as eczema and psoriasis continues to help commissioners to deliver high quality dermatological care.<\/p>

With regard to strategic clinical networks (SCNs), the first SCNs were chosen by NHS England using criteria developed with input from a broad range of stakeholders. These are initial groupings and it is expected that they will be expanded in the future. NHS England is undertaking a review of the role, purpose and function of the sub-regional infrastructure that it funds, which is focussed on supporting commissioners and providers to improve the quality of services including SCNs. This review is ongoing and is expected to reach its conclusions by the end of March.<\/p>

NHS England currently has no plans to introduce a National Clinical Director for Dermatology. However, NHS England has established a clinical reference group for specialised dermatology to bring together clinical experts, patients and carers to share best practice and improve services for patients.<\/p>

The Government has mandated Health Education England (HEE) to provide national leadership on education, training and workforce development.<\/p>

HEE sets out its commissioning intentions based upon the needs of local employers, providers, commissioners and other stakeholders who come together as members of HEE\u2019s Local Education Training Boards (LETBs).<\/p>

HEE\u2019s national workforce plan for England is an aggregate of local LETBs\u2019 plans, but the advice and input of clinical advisors and patient representatives, as well as the Royal Colleges and other stakeholders, is also listened to and considered.
