There are no plans to classify surgical fires in operating theatres as Never Events.<\/p>

Never Events are defined as serious incidents that are wholly preventable because guidance or safety recommendations that provide strong systemic protective barriers are available at a national level and should have been implemented by all healthcare providers.<\/p>

Strong systemic protective barriers are defined as barriers that must be successful, reliable and comprehensive safeguards or remedies \u2013 such as a connector that stops a medicine being given by the wrong route or a window restrictor which prevents a window being opened wide enough for a patient to get through.<\/p>

There is currently no national guidance or safety recommendations that provides such safeguards to prevent surgical fires in operating theatres. A COVID-19 vaccine will only be deployed once it has been proven to be safe and effective through robust clinical trials and approved for use by the appropriate licensing authority. There are numerous potential vaccine candidates in development, which the United Kingdom will have access to. The health, safety and wellbeing of patients, communities, individuals and teams remain our absolute priority.<\/p>

NHS England and NHS Improvement\u2019s guidance \u2018Visiting healthcare inpatient settings during the COVID-19 pandemic\u2019 has consistently been that there should be reasonable adjustments to allow certain groups of people, including people with dementia, to have a family member or friend visit them if not being present would cause the patient to be distressed. This applies to all inpatient settings.<\/p>

In addition, to support the provision of person-centred care for older people, including those with dementia, local National Health Service trusts provide hospital passport templates that can be completed before attending hospital as an inpatient or outpatient. Estimates of the number of children who suffer from \u2018undernutrition\u2019 is not available.<\/p>

The National Child Measurement Programme (NCMP) reports the number of children who are underweight in England. NCMP data for 2019/20 shows that the number of underweight children in England aged four to five years old is 3,771 out of 399,470 measured (0.9%) and the number of children aged 10-11 years old is 6,969 out of 491,138 measured (1.4%). The reasons for \u2018undernutrition\u2019, which is more generally referred to as malnutrition, are varied and complex. \u2018Undernutrition\u2019 amongst children can be both a cause and a consequence of ill health and may be linked to clinical, social and economic factors. It may develop as a result of limited access to food, a disorder or disease that results in a poor appetite, makes eating or absorbing food difficult or results in a greatly increased requirement for energy and nutrients.<\/p>
