The report of the review is due to be published in the Autumn.</p><p> </p><p>No other Tailored Reviews, which are reviews of Arms Length Bodies, are currently underway and there are no plans to conduct any other reviews as part of the current programme which concludes at the end of 2019/20.</p><p> </p><p>The Ministry of Justice does not hold a central record of reviews commissioned by the department. Further disclosure control may be completed where this alone is not sufficient.</p><p> </p><p>It is important to remember that prisoners serving IPP sentences have committed serious sexual or violent offences and remain in prison because the independent parole board have assessed their risk of serious harm to the public to be too great to warrant their release. Prisoners who have served their minimum tariff have their suitability for parole reviewed by the Parole Board at least every 2 years</p><p> </p><p>Due to improvements in IT systems, detailed statistics are available from 2015 only and therefore</p><p>pre-2015 figures for releases from IPP sentences broken down by establishment have not been included.</p><p> </p><p>Please note that:</p><p>HMP Blantyre House was temporarily taken out of use from January 2015</p><p>HMP Downview reopened in May 2016 after a period of refurbishment.</p><p>HMP Holloway closed in May 2016</p><p>HMP Kennet closed in December 2016</p><p>HMP Berwyn opened in February 2017</p><p> </p><p>HMP Peterborough is a dual purpose prison for men and women.</p><p> </p><p>'..' 	answer	answer is ministerial correction date of answer 05/07/2019 answer text <p>The table provides the rates of incidents of self-harm and self-inflicted deaths per 1,000 prisoners for the 12 month period to September 2018. On average, those who self-harmed did so 4.4 times, although a small number of prolific self-harmers have a disproportionate effect on this figure. The majority of those who self-harm in prison do so only once a year.</p><p> </p><p>The Government is taking unprecedented action to improve safety in prisons, including redoubling our efforts to prevent self-inflicted deaths and to reduce the levels of self-harm. We have recruited over 4,700 more prison officers since October 2016, and we now have the greatest number in post since early 2012. This is allowing us to implement the key worker role, allowing staff dedicated time to provide support to individual prisoners.</p><p> </p><p>We are improving support for prisoners in their early days in custody and working to improve the multi-disciplinary ACCT case management process for those at risk of suicide or self-harm.</p><p>We have rolled out a revised and improved Introduction to Suicide and Self-harm Prevention course. This is being completed by all new staff and as refresher training by all existing staff. 	answer	answer is ministerial correction date of answer 18/06/2019 answer text <p>The Government publishes quarterly statistics on self-inflicted deaths in prison, and a more detailed annual breakdown, which is available at:</p><p><a href="https://www.gov.uk/government/statistics/safety-in-custody-quarterly-update-to-september-2018" target="_blank">https://www.gov.uk/government/statistics/safety-in-custody-quarterly-update-to-september-2018</a></p><p> </p><p>Table 1.1 of the document ‘Deaths in prison custody 1978-2018’ shows the rate of self-inflicted deaths for the prison population in each calendar year. In 2018 the rate was 1.12 per 1000 prisoners.</p><p> </p><p>Table 1.11 of the same document shows the number of self-inflicted deaths among those serving sentences of imprisonment for public protection in each calendar year. In 2018 there were 5 such deaths. This was at a rate of 1.38 per 1,000 IPP prisoners. Figures such as this for rates should be considered with caution when derived from relatively small numbers of events.</p><p> </p><p>The Government is taking unprecedented action to improve safety in prisons, including redoubling our efforts to prevent self-inflicted deaths. We have recruited over 4,700 more prison officers since October 2016, and we now have the greatest number in post since early 2012. This is allowing us to implement the key worker role, allowing staff dedicated time to provide support to individual prisoners.</p><p> </p><p>We are improving support for prisoners in their early days in custody and working to improve the multi-disciplinary ACCT case management process for those at risk of suicide or self-harm.</p><p>We have rolled out a revised and improved Introduction to Suicide and Self-harm Prevention course. This is being completed by all new staff and as refresher training by all existing staff. 	answer	answer is ministerial correction date of answer 18/06/2019 answer text <p>Controllers at each privately-managed prison hold regular review meetings with the contractor against a range of performance indicators that will reflect staffing, recruitment, training, sickness, and attrition and any concerns are discussed at these meetings. Where action is needed, progress is monitored by the relevant Controller and escalated within HM Prison and Probation Service (HMPPS) where appropriate action can be taken in accordance with the contract. 	answer	answer is ministerial correction date of answer 17/06/2019 answer text <p>Attending an accredited offending behaviour programme (OBP) in custody is voluntary, meaning there are not any automatic sanctions for non-attendance. Prisoners who exhibit unacceptable behaviours whilst involved in a programme will face standard disciplinary procedures. Removal from programmes, however, can also take place due to behaviour unrelated to participation in a programme. Sanctions can be implemented through the adjudication process or in relation to Incentives and Earned Privileges (IEP). In order to collect the information requested a matching exercise using prison population and adjudication databases would be required to identify each individual prisoner who was punished under the Prison Rules. As a result, the data could only be obtained at disproportionate cost.</p><p> </p><p>Accredited Programmes can also be part of a Community Sentence or a Licence Condition, in which case offenders can be breached or recalled to custody for not being compliant.
