star this property answer text <p>Local authorities have a duty to ensure people including those with dementia receive appropriate care and support.</p><p>Adult social care is a locally delivered system therefore local authorities are locally accountable for the delivery of care and support and retain significant discretion over how their functions are exercised. Different communities require different social care provision and local authorities are best placed to understand the needs of their constituents.</p><p> </p><p>We have given local authorities access to up to £3.9 billion more dedicated funding for adult social care this year, and a further £410 million is available for adults and children’s services. The Government will set out plans to reform the social care system to ensure it is sustainable for the future at the earliest opportunity.</p><p> </p><p> </p><p><strong> </strong></p> more like this

This was 16% of the total number of people discharged or transferred within the month.</p><p> </p><p>In March 2019, there were 105 delayed discharges recorded in the AT data. star this property answer text <p>As with the vast majority of NHS services, the funding and commissioning of palliative and end of life care is a local matter, over which individual National Health Service commissioners have responsibility. Local commissioners are best placed to understand the needs of local populations and commission services to meet those needs accordingly.</p><p> </p><p>The NHS Long Term Plan, sets out significant investment and activity to improve the quality of patient care and health outcomes, including for end of life care. This includes £4.5 billion of new investment to fund expanded community multidisciplinary teams aligned with new primary care networks. Based on individual needs and choices, people identified as having the greatest risks and needs will be offered targeted support for both their physical and mental health needs, helping them to maintain independence and avoid unnecessary hospital admissions. This includes patients approaching the end of life.</p> more like this

star this property answer text <p>Departmental support for staff transitioning into retirement includes a Pre Retirement learning event, provided via Civil Service Learning, which raises awareness of available information and resources, supports proactive planning for making the most of retirement and awareness of how to claim Civil Service and state pensions.</p><p>All staff in the Department who are part of the Civil Service Pension Scheme, have access to MyCSP, the Civil Service pension provider, which provides online resources and tools, such as the Retirement Modeller and a Pension Portal and also provides individual pension advice over the phone.</p><p>The Department also offers and wherever possible, supports staff requests for partial retirement to support individual needs and also as a transition to future retirement.</p> more like this

star this property answer text <p>‘Prevention is better than cure’, published by the Department in November 2018, highlighted the important role social prescribing can play in reducing people’s isolation and improving levels of activity.</p><p>As set out in the NHS Long Term Plan, NHS England has committed to deliver at least £4.5 billion of new investment in primary medical and community health services over the next five years. star this property answer text <p>Public Health England (PHE) is working in partnership with the Department for Education to produce evidence-based resources and tools to support health visitors, early years practitioners and parents/carers in identifying and supporting children’s early speech, language and communication (SLC) needs through training guidance and an enhanced early language assessment tool. The assessment tool will incorporate parent resources to support family understanding of SLC in their children and will be rolled out from April 2020.</p><p> </p><p>Alongside this work, PHE is developing a model SLC pathway for children under five. This will ensure that professionals working with families are able to help them support their child’s SLC and how to access support if needed. The pathway will be published by September 2019.</p> more like this
