answer	answer is ministerial correction date of answer 22/03/2018 answer text <p>The Forestry Commission and the Animal and Plant Health Agency been actively managing Phytophthora ramorum and Phytophthora kernovie since 2009 to minimise spread.</p><p> </p><p>Phytophthora ramorum is found across the UK. In England most infected sites are in the south west and north west. We continue our risk-based approach to identify infection in the wider environment and when it is found we issue Statutory Plant Health Notices to fell infected larch and rhododendron.</p><p> </p><p>We still have specific measures in place that regulate the movement and processing of infected larch to prevent the spread through the trade in timber and related products. answer	answer is ministerial correction date of answer 13/03/2018 answer text <p>The standards, proficiencies, and outcomes of nursing training are the responsibility of the United Kingdom regulator, the Nursing and Midwifery Council, which is an independent statutory body.</p><p> </p><p>All nurses undertake mental health placements as part of pre-registration training and nurses in emergency departments receive additional mental health training. Staff also have access to Mental Capacity Act training as part of safeguarding arrangements.</p><p> </p><p>Trusts, as individual employers, are responsible for ensuring staff have the skills required to deliver safe and effective care. answer	answer is ministerial correction date of answer 13/03/2018 answer text <p>We do not hold all of the information requested centrally.</p><p> </p><p>NHS England has provided the attached information for the period between September 2016 and June 2017, which includes a monthly status of reported serious incidents with the following investigative status: planned, underway, completed, awaiting clearance or not yet allocated for investigation ‘blank’.</p><p> </p><p>The following points should be noted:</p><p> </p><p>- This information has been collected since August 2016, when NHS England put in place a national system for sub-regional and regional teams to escalate serious incidents via a Serious Incident Desk;</p><p>- NHS England does not hold information at a national level as to why investigations have not yet been completed. Most investigations should be completed within six months of being commissioned, however some may take longer due to the complexity of the incident; and</p><p>- NHS England does not hold information on serious investigations only led by clinical commissioning groups or trusts. answer	answer is ministerial correction date of answer 12/03/2018 answer text <p>River Basin Management Plans provide an overview of a River Basin District and give details of water bodies and their condition as well as proposed actions needed for water bodies to improve status and prevent deterioration. These include actions that water companies and other sectors need to complete to help achieve good status and prevent deterioration.</p><p> </p><p>United Utilities’ Asset Management Plan (AMP6) for the period 2015 to 2020 includes a number of schemes designed to improve water quality across the Ribble catchment. By improving the quality of waste water discharged to rivers and coastal waters from sewage treatment works and reducing the frequency and duration of discharges from storm overflows, water quality will be improved in, for example, the River Darwen, River Calder, River Hyndburn, River Yarrow and Colne Water.
