answer text <p>The total cost of the current contract for the National Diet and Nutrition Survey (NDNS), covering four years of fieldwork from 2013/14 to 2016/17 and including analysis and reporting, is £15.4 million.</p><p>The most recent NDNS report, covering diet, nutrient intake and nutritional status in United Kingdom adults and children, was published in May 2014. This report was based on data collected under the previous contract for NDNS covering fieldwork from 2008/09 to 2011/12.</p> more like this

answer text <p>HM Revenue and Customs (HMRC) does allow customers to enter their title but it does not allow customers to enter other honours. Major changes would be required to a wide range of internal HMRC IT services to capture and store this information. The cost of making such changes would be disproportionate.</p> more like this

answer text <p>DCLG is not a transactional department and only has one form on GOV.UK. This form is a contact us form, which does not ask for any titles such as Miss, Mr etc at present. We do not have any plans to add honours on to this form. However, if a member of the public wanted to add their honours following the entry of their name in the full name box they would be able to do so.</p> more like this

answer text <p>The Department for Education works closely with the Government Digital Service (GDS) to ensure the digital products and services delivered are fit for purpose and put the user first. Each of the online forms is developed in accordance with the GDS Service Standard, applying design principles and house style to ensure consistency and a satisfying user experience.</p><p> </p><p>When designing online forms, the only questions asked are those that are relevant and required and eliminate unnecessary steps and fields that are not pertinent to the purpose of the data we are collecting. At present there are no plans to include an honours question as a standard element across all of the online forms but if the data being collected requires this information to be collected then the online form will contain this particular field.</p> more like this

answer text <p>My Department provides a wide variety of online forms across multiple systems. Most of these are designed to enable web users to use perform a transaction, such as to book a driving theory test. Therefore we only ask our users the minimum information required to process their requests to enable them to complete these transactions as quickly as possible.</p><p> </p><p>I am mindful that there could be an additional cost to the taxpayer in making changes to online systems to capture and store additional data.</p><p> </p> more like this
