answer	answer is ministerial correction date of answer 13/05/2014 answer text <p>The UN Security Council adopted the renewed Mandate of the United Nations Mission for the Referendum in Western Sahara (MINURSO) on 29 April. The new mandate does not include an extension of the mission's responsibilities to include human rights monitoring in the territory, but it does welcome Morocco's recent steps to strengthen the National Council on Human Rights (CNDH), and its continuing interaction with Special Procedures of the United Nations Human Rights Council, including that planned for 2014. It also welcomes the planned visit of the Office of the High Commissioner for Human Rights (OHCHR) in 2014.</p><p>The British Government strongly encourages progress towards effective human rights monitoring in Western Sahara and the refugee camps in Tindouf. We also welcome Morocco's recent commitments to investigate complaints of human rights violations made to the National Council for Human Rights within three months, and to end military tribunal trials for civilians. answer	answer is ministerial correction date of answer 13/05/2014 answer text <p>Following the resumption of the Intergovernmental Authority on Development (IGAD)-chaired talks in Addis Ababa, a face-to-face meeting between South Sudan's President Kiir and former Vice President Riek Machar took place in Addis on 09 May. This resulted in the two leaders signing a new peace agreement, recommitting both sides to the Cessation of Hostilities Agreement (CoHA) originally signed in January this year. The new agreement also commits both parties to work towards a Transitional Government of National Unity which will oversee a new Constitution and guide the country towards elections. The agreement also reaffirms existing commitments to allow humanitarian assistance to reach affected populations on the ground.</p><p> </p><p>The Minister for Africa, my hon. answer	answer is ministerial correction date of answer 14/05/2014 answer text <p>From incidents reported to the National Reporting and Learning System (NRLS) there is evidence that the prescription of opioid medicines for all conditions, including chronic pain, have led to fatal overdoses in patients in England and Wales. However, this information cannot be used to provide evidence of abuse.</p><p> </p><p>The report <em>A review of controlled drug incidents reported to the NRLS over seven years</em> (2005 to 2011) published in the Pharmaceutical Journal in 2013 provided details of the number and types of patient safety incidents involving Controlled Drugs (CDs), including opioid medicines, reported to the NRLS. In addition incidents with reported outcomes of death and severe harm were also analysed qualitatively. A copy of the report has been placed in the Library.</p><p> </p><p>There were 72,028 incidents reported to the NRLS over the seven year period. Of 10,678 incidents of reported harm there were 54 deaths, 74 incidents of severe harm and 10,550 incidents of other harm. Incidents involving overdose of CDs accounted for 89 (69.5%) of the 128 incidents reported of serious harm (combining death and severe harm).</p><p> </p><p>Unsafe use of CDs is the number one cause of serious harm from medication incidents reported to the NRLS. NHS England is working with the Care Quality Commission and has established a clinical safety group of the National Accountable Officers for Controlled Drugs Committee.
