In every case where a breach is recorded, the scale of the breach is determined by assessing the circumstances of each case in line with our department’s disciplinary guidance. This was a loss by a 3rd party supplier working on behalf of the Office of Civil Society (OCS) collecting survey data​.</p><p> </p><p>​In ​2017​ we introduced a more robust method of incident ​reporting​.
