answer text <p>Following the resumption of the Intergovernmental Authority on Development (IGAD)-chaired talks in Addis Ababa, a face-to-face meeting between South Sudan's President Kiir and former Vice President Riek Machar took place in Addis on 09 May. This resulted in the two leaders signing a new peace agreement, recommitting both sides to the Cessation of Hostilities Agreement (CoHA) originally signed in January this year. The new agreement also commits both parties to work towards a Transitional Government of National Unity which will oversee a new Constitution and guide the country towards elections. The agreement also reaffirms existing commitments to allow humanitarian assistance to reach affected populations on the ground.</p><p> </p><p>The Minister for Africa, my hon. Friend the Member for Boston and Skegness (Mark Simmonds), made a statement on 10 May welcoming the agreement as an important and long overdue step, and urging the immediate implementation of the peace deal.</p><p> </p> more like this

answer text <p>From incidents reported to the National Reporting and Learning System (NRLS) there is evidence that the prescription of opioid medicines for all conditions, including chronic pain, have led to fatal overdoses in patients in England and Wales. However, this information cannot be used to provide evidence of abuse.</p><p> </p><p>The report <em>A review of controlled drug incidents reported to the NRLS over seven years</em> (2005 to 2011) published in the Pharmaceutical Journal in 2013 provided details of the number and types of patient safety incidents involving Controlled Drugs (CDs), including opioid medicines, reported to the NRLS. In addition incidents with reported outcomes of death and severe harm were also analysed qualitatively. A copy of the report has been placed in the Library.</p><p> </p><p>There were 72,028 incidents reported to the NRLS over the seven year period. Of 10,678 incidents of reported harm there were 54 deaths, 74 incidents of severe harm and 10,550 incidents of other harm. Incidents involving overdose of CDs accounted for 89 (69.5%) of the 128 incidents reported of serious harm (combining death and severe harm).</p><p> </p><p>Unsafe use of CDs is the number one cause of serious harm from medication incidents reported to the NRLS. NHS England is working with the Care Quality Commission and has established a clinical safety group of the National Accountable Officers for Controlled Drugs Committee. This group is providing further guidance to Accountable Officers for Controlled Drugs and healthcare practitioners on safer use of controlled drugs.</p>

answer text <p>People should expect the right support to help them manage their long-term conditions, including chronic pain, and through the Mandate we have asked NHS England to make measurable progress towards making the National Health Service among the best in Europe at supporting people with ongoing health problems to live healthily and independently, with much better control over the care they receive.</p><p> </p><p>NHS England and its partners are working towards the implementation of the House of Care model, which is designed to support the delivery of person-centred, coordinated care, which enables individuals to make informed decisions which are right for them, and empower them to self-care.</p><p> </p><p>NHS England is working to produce with patients and carers a supported self-management guide for people with complex care needs and frailty by June 2014 and is developing practical tools and commissioning guidance to support the delivery of the Mandate commitment for everyone with a long term condition being offered a personalised care plan by April 2015.</p><p> </p>

answer text <p>On the evening of 1 April, the Spanish state research vessel, Angeles Alvarino, entered British Gibraltar Territorial Waters (BGTW). The vessel was shadowed by Royal Navy Gibraltar Squadron and Royal Gibraltar Police vessels. The Angeles Alvarino departed BGTW after approximately three hours. The Government publicly summoned the Spanish Ambassador to the UK on 2 April to make a formal diplomatic protest. In his statement of the same day, the Minister for Europe, my Rt. Hon. Friend the Member for Aylesbury (Mr Lidington), made clear that not only were the actions of the survey vessel unlawful, but the dangerous manoeuvring of the accompanying Spanish Guardia Civil vessel presented a significant safety concern on the waters.</p><p>Although unlawful, incursions are a violation of British sovereignty, not a threat to it. They do not weaken or undermine the legal basis in international law for British sovereignty over Gibraltar including British Gibraltar Territorial Waters.</p><p>Maritime security in Gibraltar is provided by the Royal Navy's Lifespan Patrol Vessels, HMS SCIMITAR and HMS SABRE, and up to three Rigid Hull Inflatable Boats (RHIBs), all operated by the Royal Navy Gibraltar Squadron. The Gibraltar Defence Police have two 15 metre launches and three Arctic 24 RHIBs for maritime force protection. Ordinarily three police officers are assigned to marine duties, drawing on a cadre of some 20 personnel. Larger Royal Navy ships visit Gibraltar regularly in relation to operational and training activity, reflecting its utility as a permanent joint operating base.</p><p>The Royal Gibraltar Police is responsible for law enforcement in Gibraltar. Their Marine Section has two patrol vessels, one of them with a small deployable RHIB onboard, three RHIB interceptors, one ‘jet boat' and three jet skis.</p><p>All elements of the situation, including the maritime security capabilities available to the Royal Navy Gibraltar Squadron, are kept under review. We are ready to provide additional assets to the Squadron or augment our broader maritime posture as necessary.</p>
