answer text <p>On 25 October 2018, 2,640 prisoners were being supported and managed through the Assessment, Care in Custody and Teamwork (ACCT) process.</p><p> </p><p>The number of ACCT documents opened in each of the last five years is:</p><p>2013 33716</p><p>2014 37197</p><p>2015 43714</p><p>2016 48101</p><p>2017 49298</p><p>These are the numbers of occasions on which an ACCT was opened. Because there are many cases of individual prisoners being supported using ACCT more than once within a year, the figures for each year are considerably higher than the number of prisoners who have been supported during that year.</p><p> </p><p>The Government takes very seriously its responsibility to keep staff and prisoners safe, and we are committed to reducing the incidence of self harm, suicide and violence across the estate. This is why we have established a prison safety programme through which we are taking forward a comprehensive set of actions to improve safety in custody, including: rolling out revised and improved training for staff in assessing and managing the risk of suicide and self-harm amongst prisoners (which has already reached more than 17,000 staff); improving support for prisoners in their early days in custody; revising the ACCT case management process for those identified as being at risk; and renewing our partnership with the Samaritans by confirming a further three years' grant funding for their valuable Listeners Scheme.</p>

This includes not only suicides but also accidental deaths resulting from the person’s own actions. answer text <p>Data about deaths of offenders under probation supervision can be found at https://www.gov.uk/government/statistics/deaths-of-offenders-in-the-community-annual-update-to-march-2017. The next set of figures, up to March 2018, is expected to be published in October.</p><p> </p><p>Not every offender released from custody is under probation supervision, so some deaths will not be shown in those figures. The statistical data about deaths that is available from other sources does not record whether or not the deceased had previously been imprisoned.</p> more like this

answer text <p>Yes. All staff in contact with prisoners are required to receive training on suicide and self-harm prevention. The Introduction to Suicide and Self-Harm (SASH) Prevention course is delivered to new prison staff as part of their induction, and to existing prison staff as part of their ongoing refresher training as required. The course comprises six modules, including mental health awareness training.</p><p> </p><p>The course aims to ensure that the case management support process for prisoners at risk of self-harm or suicide (known as ‘Assessment Care in Custody and Teamwork’ or ACCT) is carried out effectively in prisons, to provide a safe and decent environment for all.</p> more like this

answer text <p>This information is not collected centrally.</p><p> </p><p>Every self-inflicted death in prison custody is independently investigated by the Prisons and Probation Ombudsman (PPO) and is the subject of a coroner’s inquest. We take the findings of these investigations very seriously. Her Majesty’s Prison and Probation Service works with healthcare partners to ensure that action is taken to address the recommendations made by the PPO, and any matters of concern brought to our attention by coroners, including any that relate to the provision of medication or therapies for mental health conditions.</p> more like this
