answer	answer is ministerial correction date of answer 18/04/2016 answer text <p>NHS England continues to work with Muscular Dystrophy UK through the Bridging the Gap Project, which aims to ensure people with neuromuscular conditions, along with their families, play a leading role in the development and commissioning of the services they use.</p><p>NHS England commissions some services for patients with neuromuscular conditions nationally as part of its remit to deliver specialised neurological services. However, the majority of services are provided outside of specialised centres and are the responsibility of clinical commissioning groups to provide, should they deem it clinically appropriate. This includes services such as hydrotherapy.</p><p>In 2012 Muscular Dystrophy UK, working with the Department’s National Institute for Health Research (NIHR) issued a call for research proposals into the clinical effectiveness of hydrotherapy in maintaining physical function in people with Duchenne muscular dystrophy (DMD). The call was made because there is little evidence to support the use of hydrotherapy in relation to DMD and other neuromuscular disorders. answer	answer is ministerial correction date of answer 18/04/2016 answer text <p>In national service model and <em>Building the right support </em>published in October 2015 NHS England, the Local Government Association and the Association of Directors of Adult Social Services set out how areas would be supported to deliver lasting change to people with a learning disability and/or autism who display behaviour that challenges.</p><p>To develop community capacity, clinical commissioning groups (CCGs), local authorities and NHS England’s specialised commissioners have formed 48 transforming care partnerships (TCPs) to plan for the future. TCPs have been asked to use the total sum of money they spend as a whole system on people with a learning disability and/or autism to deliver care in a different way to achieve better results. This includes shifting money from some services (such as inpatient care) into others (such as community health services including mental health services or individual packages of support). The costs of the future model of care will therefore be met from the total current envelope of spend on health and social care services for people with a learning disability and/or autism.</p><p>During a phase of transition, commissioners will need to invest in new community support before closing inpatient provision. To support them to do this NHS England will make available up to £30 million of transformation funding over three years, to be matched by CCGs, and £15 million in capital funding. For patients with long term conditions (LTCs), including musculoskeletal (MSK) conditions, we have asked the NHS to improve the care and support of patients, helping them to live healthily and independently, with much better control over the care they receive.</p><p> </p><p>In response, NHS England has set out a range of actions designed to deliver this, central to which was implementation of the House of Care model, which is designed to support the delivery of person-centred, coordinated care. The House of Care enables individuals to make informed decisions about their treatment and empowers them to self-manage their LTCs in partnership with health and care professionals. NHS England has provided data, tools and guidance to support local services in identifying those most likely to benefit from a care planning approach. Specifically on MSK conditions, the National Clinical Director for MSK, Peter Kay, has been working in partnership with the Arthritis and Musculoskeletal Alliance, to develop new MSK clinical networks across England to build consensus on the way forward for models of care.</p><p> </p><p>In terms of assessment, the first annual report of the National Clinical Audit of Rheumatoid and Early Inflammatory Arthritis, commissioned on behalf of NHS England by the Healthcare Quality Improvement Partnership (HQIP), was published on 22 January 2016. This report, which includes data from 1 February 2014 to 30 April 2015, assesses the quality of care by specialist rheumatology services using criteria derived from sources such as the Rheumatoid Arthritis Quality Standard, published by the National Institute for Clinical Excellence in June 2013. The report identifies that although most services offer prompt educational support and agree targets for treatment with their patients, performance against criteria for referral and assessment could be improved. Since the audit, HQIP has reported that a number of trusts have successfully reconfigured their services in order to improve patient care.
