answer text <p>The Government assesses data from the Human Tissue Authority (HTA) on serious incidents at quarterly accountability meetings.</p><p>When these incidents occur they are upsetting for the families of those involved; although they are rare, when they do happen we are assured that the HTA work with establishments to ensure that a thorough investigation takes place, and that improvements are made to reduce the risk of similar incidents happening again.</p> more like this

answer text <p>Due to the way that agency expenditure was measured across legacy organisations prior to the creation of NHS Improvement, comparable figures are only available from 2012-13 onwards.</p><p>The following table shows that after a 40% increase in total agency expenditure between 2012-13 and 2015-16, new measures introduced to reduce agency spend across the National Health Service have been successful. Since their introduction, agency spend has fallen from a peak of £3.6 billion in 2015-16 to £2.4 billion in 2017-18 – a £1.2 billion reduction.</p><p>We recognise that there is still more to be done in this area, and we are committed to reducing trusts’ reliance on expensive agency workers, whilst ensuring they can meet their temporary staffing needs. answer text <p>The Human Fertilisation and Embryology Authority (HFEA) has strengthened its requirements for how clinics should take consent from patients and partners in 2016.</p><p>The new edition of the HFEA’s Code of Practice, to be published shortly, will provide further guidance on the processes clinics should have in place to ensure consent is informed, taken properly and given by the right person. The guidance states that consent should be given at the clinic, where possible, and where this is not possible, that there should be a documented process in place for ensuring consent has been given by the right person. In particular, clinics need to be satisfied about the patients’ legal relationships to each other and, where the partner of a patient has not visited the clinic or does not return for subsequent treatment, the clinic should take reasonable steps to find out if the partner still consents to treatment.</p> more like this

answer text <p>The Department is aware of the Centre for Social Justice’s recent report into supporting fathers during the perinatal period and early parenthood, published in December 2018. The Department recognises the vital role fathers can often play during pregnancy and early years, as well as the fact that every family is different.</p><p>We welcome the high levels of engagement fathers currently have with various aspects of the health service, as cited in the report, including the estimated 94% of new fathers that have attended one or more antenatal appointments, and almost all attending both ultrasound scans and the birth itself.</p><p>The Maternity Transformation Programme is working to make maternity care more personalised including responding to the needs of fathers as well as mothers. Postnatal and perinatal mental health care, maternity care and personalised care plans are recognised as important for the whole family, including fathers, and they should therefore be involved where appropriate.</p><p>Furthermore, the Healthy Child Programme states that the father’s contribution should be one of the key topics to be covered during a health and development review by a health visitor. It emphasises the importance of ensuring that contact with the family routinely involves and supports fathers, including non-resident fathers.</p>

answer text <p>The Human Fertilisation and Embryology Authority (HFEA) has advised that it is a requirement for all its licensed centres to report cases of ovarian hyperstimulation syndrome (OHSS) that require a hospital admission and have a severity grading of severe or critical. Some centres also report cases of mild or moderate OHSS.</p><p>Between 1 October 2013 and 31 October 2018 there were 792 OHSS cases reported requiring hospital admission (391 of which were classified as severe and 401 classified as mild/moderate). HFEA does not hold data on whether these admissions were out of hours or via emergency admission. Following the reports of cases of severe or critical OHSS, the centres are required to conduct a root cause analysis and submit a report to the HFEA. The HFEA publishes an annual report on incidents, of which OHSS cases form a part.</p><p>From April 2018 the HFEA has required its licensed centres to provide it with more information about treatment provided to patients in cases of severe OHSS. This information will be collected for two years and will then be analysed.</p><p>The HFEA has also advised that following discussions with key stakeholders it has reached a view that the Hospital Episode Statistics data may include a mislabelling of cases of OHSS. A paper presented to Authority members in January 2018 sets out the work undertaken to examine the reporting of severe and critical OHSS. A copy of <em>Ovarian Hyperstimulation Syndrome</em> is attached. HFEA does not hold estimates of the number of hospital admissions that may have been incorrectly coded for OHSS.</p><p>HFEA is continuing to work to better understand the incidence of OHSS, reduce its occurrence and improve reporting.</p><p><strong> </strong></p>
