answer text <p>The responsibility for commissioning severe and complex obesity for Tier 4 services will transfer to CCGs in April 2016. This is one of a number of changes in commissioning responsibility which will be managed between NHS England and CCGs. NHS England will be leading on the contracting for services for 2016/17 to facilitate the smooth handover from 1 April 2016.</p><br /><p>NHS England is addressing preparedness, both around the technical and financial transfer of budget and contracting responsibility, and supporting CCGs in commissioning of the clinical pathway.</p><br /><p>The technical impact of these changes is being modelled and formal agreement within NHS England will occur in mid-December. The formal transfer will be part of the annual national Planning Guidance to commissioners, including CCGs that will be issued in early 2016.</p><br /> <br /> more like this

answer text <br /><p>CCGs will have the full commissioning responsibility for the obesity services within their remit to set standards for services for their population and to make clinical commissioning policy decisions in terms of patient eligibility with effect from April 2016.</p><br /><p>NHS England is supporting the transfer of commissioning responsibilities to CCGs by providing clinical guidance on standards for the adult obesity surgery pathway. This guidance is in development, supported by national clinical experts.</p><br /><p>The draft clinical commissioning policy includes reference to the relevant National Institute for Health and Care Excellence guidance to support CCGs in determining patient eligibility to obesity surgery. The draft service specification will reflect best practice and describes the standard providers will need to meet for the surgical service.</p> more like this

answer text <p>Due to feedback during the consultation for the proposal, the responsibility for commissioning renal dialysis services is not transferring to clinical commissioning groups (CCGs) in April 2016.</p><br /><p>NHS England is supporting the transfer of commissioning responsibilities for obesity surgery services to CCGs by providing technical and operational guidance to commissioners, including undertaking a stocktake of Tier 3 and Tier 4 interfaces within regions to assist CCGs. This guidance is in development. Communications are also supported through national and local meetings between NHS England and CCG representatives on shared issues within the commissioning agenda.</p> more like this

answer text <p>The expert forum - the Cardiovascular Disease (CVD) Collaborative Strategy Group - has representation from key CVD stakeholders including NHS England, Public Health England, Department of Health and third sector organisations such as the British Heart Foundation, Diabetes UK and the National Kidney Federation. It provides leadership to the National Health Service in developing approaches to improve the prevention, early diagnosis and management of CVD as highlighted in the CVD outcomes strategy.</p><br /><p>As part of its work, the group is exploring approaches to support the earlier diagnosis of atrial fibrillation, high blood pressure, heart failure and valve disease and to improve outcomes from out of hospital cardiac arrest. It is also considering how it can support wider strategic developments arising from the Five Year Forward View, such as the development of seven day services.</p><br /><p>In addition, NHS England’s National Clinical Director for Heart Disease continues towork with partners on familial hypercholesterolemia (FH). Working with Public Health England, they aim to identify more families with FH and address the importance of cholesterol on general as a risk factor for CVD. The National Clinical Director alsochairs an FH Steering Group, which brings together relevant stakeholders, and supportsStrategic Clinical Networks around the country so that good practice can be shared more widely.</p><br />

answer text <p>The Department funded Infertility Network UK (INUK) to develop advice to NHS Commissioners on standardising eligibility criteria for in-vitro fertilisation (IVF) treatment. The Department has consistently encouraged NHS Commissioners to have regard to this advice. The INUK advice is listed as a resource for clinical commissioning groups (CCG) in the NHS England “Commissioning Fertility Services Factsheet”.</p><p><strong></strong></p><p>National Institute for Health and Care Excellence (NICE) guidelines recommend that eligible couples should receive three full cycles of IVF treatment. The Government has made clear that blanket restrictions on treatment are unacceptable and all decisions on treatment should be made by doctors based on a patient’s individual clinical needs and in line with NICE Guidelines.</p><br /><p>While we have not made an assessment of access to IVF treatment, we are aware of the annual survey by Fertility Fairness. I have written to stakeholders, including Fertility Fairness, and invited them to discuss what could be done to improve the provision of IVF services along with representatives of NHS England and Monitor.</p><br /> <br />

This collaborative continues to coordinate delivery of the work which was initiated in the CVD Outcomes Strategy.</p><br /><p>As part of its work on the seven days services programme, NHS Improving Quality has surveyed 24/7 diagnostic service provision (including cardiac physiology) and has announced early adopter seven day sites. One of these adopter sites focuses on provision of seven day services to support cardiac interventions and improved bed usage.
