answer	answer is ministerial correction date of answer 10/12/2015 answer text <p>The responsibility for commissioning severe and complex obesity for Tier 4 services will transfer to CCGs in April 2016. This is one of a number of changes in commissioning responsibility which will be managed between NHS England and CCGs. NHS England will be leading on the contracting for services for 2016/17 to facilitate the smooth handover from 1 April 2016.</p><br /><p>NHS England is addressing preparedness, both around the technical and financial transfer of budget and contracting responsibility, and supporting CCGs in commissioning of the clinical pathway.</p><br /><p>The technical impact of these changes is being modelled and formal agreement within NHS England will occur in mid-December. answer	answer is ministerial correction date of answer 10/12/2015 answer text <br /><p>CCGs will have the full commissioning responsibility for the obesity services within their remit to set standards for services for their population and to make clinical commissioning policy decisions in terms of patient eligibility with effect from April 2016.</p><br /><p>NHS England is supporting the transfer of commissioning responsibilities to CCGs by providing clinical guidance on standards for the adult obesity surgery pathway. This guidance is in development, supported by national clinical experts.</p><br /><p>The draft clinical commissioning policy includes reference to the relevant National Institute for Health and Care Excellence guidance to support CCGs in determining patient eligibility to obesity surgery. This guidance is in development. answer	answer is ministerial correction date of answer 30/11/2015 answer text <p>The expert forum - the Cardiovascular Disease (CVD) Collaborative Strategy Group - has representation from key CVD stakeholders including NHS England, Public Health England, Department of Health and third sector organisations such as the British Heart Foundation, Diabetes UK and the National Kidney Federation. It provides leadership to the National Health Service in developing approaches to improve the prevention, early diagnosis and management of CVD as highlighted in the CVD outcomes strategy.</p><br /><p>As part of its work, the group is exploring approaches to support the earlier diagnosis of atrial fibrillation, high blood pressure, heart failure and valve disease and to improve outcomes from out of hospital cardiac arrest. It is also considering how it can support wider strategic developments arising from the Five Year Forward View, such as the development of seven day services.</p><br /><p>In addition, NHS England’s National Clinical Director for Heart Disease continues towork with partners on familial hypercholesterolemia (FH). Working with Public Health England, they aim to identify more families with FH and address the importance of cholesterol on general as a risk factor for CVD. The Department has consistently encouraged NHS Commissioners to have regard to this advice. The INUK advice is listed as a resource for clinical commissioning groups (CCG) in the NHS England “Commissioning Fertility Services Factsheet”.</p><p><strong></strong></p><p>National Institute for Health and Care Excellence (NICE) guidelines recommend that eligible couples should receive three full cycles of IVF treatment. The Government has made clear that blanket restrictions on treatment are unacceptable and all decisions on treatment should be made by doctors based on a patient’s individual clinical needs and in line with NICE Guidelines.</p><br /><p>While we have not made an assessment of access to IVF treatment, we are aware of the annual survey by Fertility Fairness. These include addressing areas such as diabetes, high blood pressure and cholesterol together with the further development of NHS Health Checks. NHS England is also working with partners to support actions that promoteearlier diagnosis of conditions such as atrial fibrillation, heart failure and valve disease and improved survival from out of hospital cardiac arrest.</p><p>NHS England also hosts an expert forum which brings together the relevant National Clinical Directors, the main relevant national charities, the National CVD Intelligence Network, PHE and the Department. This collaborative continues to coordinate delivery of the work which was initiated in the CVD Outcomes Strategy.</p><br /><p>As part of its work on the seven days services programme, NHS Improving Quality has surveyed 24/7 diagnostic service provision (including cardiac physiology) and has announced early adopter seven day sites. One of these adopter sites focuses on provision of seven day services to support cardiac interventions and improved bed usage.
