answer text <p>Every unexpected death of a patient in a mental health hospital is a matter of concern. In March 2015 NHS England issued their <em>Serious Incident Framework</em>. This gives guidance on good practice in responding to patient deaths, including those of people detained under the Mental Health Act 1983. This includes guidance on the circumstances in which National Health Service organisations should set up an independent investigation, in addition to the Coroner’s inquest. answer text <p>Smoking continues to be the leading cause of premature death and health inequality in England, placing a significant burden on the National Health Service.</p><br /><p>The Department remains dedicated to the delivery of effective and robust tobacco control and has committed to developing a new tobacco control strategy to further reduce the prevalence of smoking, working with NHS England and Public Health England (PHE) to identify interventions that will make the best contributions to achieving this.</p><br /><p>Training material and guidance has been developed to support health professionals deliver effective interventions to pregnant women who smoke.</p><p><strong></strong></p><p>PHE will continue to support local authorities by developing and distributing information and advice for the commissioning of effective smoking cessation services.</p> more like this

answer text <br /><p>The extension of direct access to diagnostic tests for primary care is an issue for clinical commissioning groups and local commissioners. Although the Independent Cancer Taskforce report did estimate that some additional diagnostic capacity was needed, it did not consider possible increases in capacity required specifically as a result of increasing general practitioner (GP) direct access to testing.</p><br /><p>The Taskforce’s five-year strategy recommends improvements across the cancer pathway, with the aim of improving survival rates, and we will work with the NHS, charities and patient groups to deliver it. We have already announced funding of up to £300 million a year by 2020 to increase diagnostic capacity to meet the new target that patients will be given a definitive cancer diagnosis, or the all clear, within 28 days of being referred by a GP.</p><br /><p>In addition, the recently updated National Institute for Health and Care Excellence referral guidelines for suspected cancer also recommend GPs refer direct-to-test in a number of different circumstances. answer text <p>NHS England’s commissioning intentions recognise that following their establishment in August 2015, Operational Delivery Networks (ODNs) will need to continue to be developed as a mechanism for ensuring equitable access to expert multidisciplinary team care and treatment for hepatitis C. ODNs are working to develop their plans, based on the published service specification and with the support of local specialised commissioning teams and regional Clinical Directors of specialised commissioning. Work is also underway to establish by February 2016 a national network of ODN clinical leads to share good practice. The formation of ODNs provides an opportunity for local collaborative commissioning conversations about hepatitis C pathways beyond the remit of specialised or health and justice services. In addition, the draft hepatitis C improvement framework includes a numbers of areas pertinent to ODN operation and will therefore support further development of their role.</p> more like this
