answer text <p>Following the announcement in June 2018, the Prime Minister has asked the National Health Service to develop a long term plan for the service. The plan will set out how the NHS will develop over the coming years, underpinned by a five-year funding offer which will see the NHS budget grow by over £20.5 billion a year in real terms by 2023-24. It provides an excellent opportunity to look at how HIV and other services can be further improved over the next decade. The plan is currently in development and will be published later in the year.</p> more like this

PHE funds HIV Prevention England, a consortium led by the Terrence Higgins Trust, to undertake national HIV prevention campaigns which aim to promote a variety of prevention approaches including combatting HIV stigma and discrimination, increasing condom use and access to HIV testing and raising awareness of treatment options that prevent either acquisition or transmission of infection.</p> more like this

answer text <p>NHS England is responsible for commissioning high quality HIV treatment and care, and therefore for addressing recommendations from the British HIV Association national audit 2015. NHS England published a service specification ‘Specialised HIV Services for Adults (Outpatient and Inpatient Services)’ in 2013. The aim is to provide specialist assessment and ongoing management of HIV and associated conditions in order to support patients to stay well (reduced mortality and morbidity) and to reduce the risk of onward transmission of HIV. The service aims to ensure that the outcomes, wellbeing and quality of life of adults with HIV are maximised. answer text <p>Information on the number of patients living with HIV who were offered and received an annual health assessment for cardiovascular disease, renal impairment, diabetes and bone health in each NHS England region is not collected centrally. An ongoing assessment for cardiovascular disease, renal impairment, diabetes and bone health is part of the core clinical and therapeutic management of patients with HIV.</p><p> </p><p>NHS England has published a service specification for HIV services which defines the standards of care expected from organisations funded by NHS England to provide specialised care. The service specification refers to the British HIV Association routine investigation and monitoring guidelines, which set the clinical standard for clinical assessments and tests. These guidelines include annual monitoring for co-morbidities and are subject to local and national clinical audit. answer text <p>Estimates of undiagnosed HIV infection are not available by Nomenclature of Territorial Units for Statistics (NUTS) 1 region.</p><p><strong> </strong></p><p>There may be some regional differences in treatment rates or drugs used based on variations in patient demographics or the available drug contracts.</p><p> </p><p>Regional HIV networks determine prescribing guidelines which align with national commissioning policies for antiretroviral therapies. These guidelines are overseen by the HIV Clinical Reference Group – which includes strong patient representation - who provide clinical advice on HIV treatment issues and raise concerns from the community should there be perceived issue around equitable access.</p><p> </p><p>Working closely with Public Health England (PHE), NHS England has produced a clinical dashboard that summarises clinical performance at the trust level. PHE prepares and analyses all the data that feeds into the Clinical Reference Group HIV dashboard. Two dashboard indicators measure anti-retroviral therapy coverage among all people accessing HIV care and among people with a low CD4 count (indicative of immunosuppression). Further information on the dashboard is available here:</p><p> </p><p><a href="https://www.england.nhs.uk/commissioning/spec-services/npc-crg/spec-dashboards/" target="_blank">https://www.england.nhs.uk/commissioning/spec-services/npc-crg/spec-dashboards/</a></p><p> </p><p> </p><p>Data are not available by NUTS for the proportion of people living with diagnosed HIV infection who are not receiving antiretroviral therapy treatment.</p><p> </p><p>Antiretroviral therapy is now so effective that people receiving treatment can expect to have a near normal life expectancy. Consequently, the proportion of people receiving treatment continues to rise each year since death rates remain low. The higher antiretroviral therapy coverage rate observed in some regions is partially due to the longer history of the epidemic in these areas.</p><p> </p><p> </p><p>British HIV Association guidelines have progressively reduced the threshold at which patients may begin antiretroviral therapy. In 2018, NHS England announced routine commissioning of ‘Immediate Access to Antiretroviral Therapy’ from 1 April 2018.</p><p> </p><p><a href="https://www.england.nhs.uk/publication/clinical-commissioning-policy-immediate-antiretroviral-therapy-for-treatment-of-hiv-1-in-adults-and-adolescents/" target="_blank">https://www.england.nhs.uk/publication/clinical-commissioning-policy-immediate-antiretroviral-therapy-for-treatment-of-hiv-1-in-adults-and-adolescents/</a></p><p> </p><p>This enables all patients to access treatment at the point of diagnosis or at the time the patient wishes to commence treatment. It has also clarified the position on the funding of antiretroviral therapy for immediate treatment among patients newly diagnosed with HIV.</p><p> </p><p>NHS England and PHE keep under review data on the rates of uptake of antiretroviral treatment and will continue to do so to identify any unwarranted variation.</p>
