answer text <p>In February 2014 NHS England announced an extension to the care.data programme to listen to the views and concerns of patients and health professionals about the programme. During the listening exercise neither type of objection was implemented as no data was extracted. This means that information for services such as cancer screening, electronic prescriptions and e-referrals is currently flowing and will continue to do so.</p><p> </p><p> </p><p> </p><p>The Health and Social Care Information Centre is committed to ensuring no patient suffers any adverse impact on their direct care through an inappropriate implementation of an objection.</p><p> </p> more like this

answer text <p>The NHS Litigation Authority (NHS LA) handles clinical negligence claims on behalf of its members under the Clinical Negligence Scheme for Trusts (CNST). Currently, all NHS trusts in England are members of the CNST.</p><p> </p><p> </p><p> </p><p>A search of the NHS LA’s claims’ database shows that less than five claims for clinical negligence involving treatment for trigeminal neuralgia have been submitted to the NHS LA from all members of the CNST during the last 10 years.</p><p> </p><p> </p><p> </p><p>I hope my hon. answer text <p>The National Reporting and Learning System identified five incidents over the past two years, which had been reported by three different National Health Service trusts.</p><p> </p><p> </p><p> </p><p>Three incidents were reported in 2013 of the following nature:</p><p> </p><p> </p><p> </p><p>Intra-procedural problem: stereotactic radiotherapy</p><p> </p><p> </p><p> </p><p>- Treatment dosage issue (reported as no harm)</p><p> </p><p> </p><p> </p><p>Intra-procedural problem: stereotactic radiosurgery</p><p> </p><p> </p><p> </p><p>- Localisation markers not correctly placed (reported as no harm); and</p><p> </p><p>- Beam positioning issue (reported as no harm).</p><p> </p><p> </p><p> </p><p>Two incidents were reported in 2014 of the following nature:</p><p> </p><p> </p><p> </p><p>Intra-procedural problem: stereotactic radiotherapy</p><p> </p><p> </p><p> </p><p>- Beam positioning issue (reported as low harm); and</p><p> </p><p>- Near miss wrong side stereotactic radiotherapy treatment due to transcription error (reported as no harm).</p><p> </p><p> </p><p> </p><p>None of these incidents gave details of the machine involved.</p><p> </p><p> </p><p> </p>

The initial capital cost of each scheme is shown in Column R and the annual revenue payment in the columns headed ‘Unitary Charge Payment’.</p><p> </p><p> </p><p> </p><p>The Treasury table shows that for the 105 NHS PFI schemes that have been signed, the estimated total revenue payments (over the lifetime of their contracts) are £79.0 billion. The revenue payment figures include not just the financing costs (debt repayment and interest) for initial construction but also the costs of all the other services such as building maintenance and support services (cleaning, catering, portering etc) provided over the lifetime of the contract. The payments are subject to meeting agreed performance and quality standards and include an annual uprate assumption for inflation of 2.5%.</p><p> </p><p> </p><p> </p><p>Over the last three years the annual total repayments have been as follows:</p><p> </p><p> </p><p> </p><p>2011/12 - £1,582 million</p><p> </p><p>2012/13 - £1,733 million</p><p> </p><p>2013/14 - £1,806 million</p><p> </p>

answer text <p>The Food Standards Agency (FSA) is a non-ministerial department and asked the National Audit Office to undertake an efficiency review in order to identify whether it was delivering the official controls required for the United Kingdom meat industry under European and UK legislation as efficiently as possible. The review took place early in 2013 and encompassed both frontline operational and support functions. answer text <p>Researchers at the National Institute for Health Research (NIHR) Biomedical Research Centre for Mental Health published findings in 2010 in the <em>British Journal of Psychiatry</em> that diabetes mellitus increases not only the risks of dementia and mild cognitive impairment but also the risk of progression from such impairment to dementia.</p><p> </p><p> </p><p> </p><p>The NIHR is currently investing nearly £0.3 million in systematic reviews of interventions which target modifiable risk factors for dementia. This evidence synthesis will encompass vascular risk factors such as diabetes.</p><p> </p> more like this

answer text <p>Auditing of hand hygiene compliance is a local responsibility, and the Department does not collect this information centrally.</p><p> </p><p> </p><p> </p><p><em>The Health and Social Care Act 2008 Code of Practice on the prevention and control of infections and related guidance</em> sets 10 compliance criteria to manage healthcare associated infections. This includes one covering infection prevention and control which refers to hand hygiene and recommends that providers undertake hand hygiene audits.</p><p> </p><p> </p><p> </p><p>The hand hygiene provisions appear to be operating satisfactorily and it is therefore not proposed that provisions relating to guidance on hand hygiene are changed as part of the revisions to the Code of Practice, which is currently out for consultation.</p><p> </p> more like this
