FAEs are counted against the year or month in which the admission episode finishes. Admissions do not represent the number of patients, as a person may have more than one admission within the period.</li><li>The primary diagnosis is the first of up to 20 (14 from 2002-03 to 2006-07 and 7 prior to 2002-03) diagnosis fields in the Hospital Episode Statistics (HES) data set and provides the main reason why the patient was admitted to hospital.</li><li>The ICD-10 code for Rickets is: E55.0 Rickets, active.</li><li>The Westminster Parliamentary Constituency code for each postcode.</li><li>The clinical commissioning group (CCG) containing the patient’s normal home address. This does not necessarily reflect where the patient was treated as they may have travelled to another area for treatment.</li><li>The strategic health authority (SHA) or primary care trust (PCT) containing the patient’s normal home address. This does not necessarily reflect where the patient was treated as they may have travelled to another SHA/PCT for treatment.</li><li>A change in methodology in 2011-12 resulted in an increase in the number of records where the PCT or strategic health authority (SHA) of residence was unknown. From 2006-07 to 2010-11 the current PCT and SHA of residence fields were populated from the recorded patient postcode. In order to improve data completeness, if the postcode was unknown the PCT, SHA and country of residence were populated from the PCT/SHA value supplied by the provider. From April 2011-12 onwards if the patient postcode is unknown the PCT, SHA and country of residence are listed as unknown.</li><li>HES figures are available from 1989-90 onwards. Changes to the figures over time need to be interpreted in the context of improvements in data quality and coverage (particularly in earlier years), improvements in coverage of independent sector activity (particularly from 2006-07) and changes in NHS practice. For example, apparent reductions in activity may be due to a number of procedures which may now be undertaken in outpatient settings and so no longer include in admitted patient HES data. Conversely, apparent increases in activity may be due to improved recording of diagnosis or procedure information.</li><li>Note that Hospital Episode Statistics (HES) include activity ending in the year in question and run from April to March, e.g. 2012-13 includes activity ending between 1 April 2012 and 31 March 2013.</li></ol><p><strong> </strong></p><p><strong> </strong></p><p><strong> </strong></p><p><strong> </strong></p><p><strong> </strong></p><p><strong> </strong></p>

answer text <p>Any death of a young person in an inpatient service is fully investigated and learned from.</p><p>The systems in place for reporting and reviewing deaths were developed at different times and fulfil different functions. These include:</p><p>- Serious Incident Review for each serious untoward incident;</p><p>- Child Deaths Overview Panels which are carried out at local level into every child death and reported to the Department for Education;</p><p>- Coroners can write to the Secretary of State (or anyone else they feel have lessons to learn) to draw his attention to specific issues of concern after the death of a child; and</p><p>- The Care Quality Commission requires providers to report the death of any detained patient (though few children and young people are detained under the Mental Health Act).</p><p>Prison policy is a matter for the Ministry of Justice.</p> more like this

answer text <p>Progress on implementing recommendation 10, which is to build the mental health intelligence capability through the National Mental Health Intelligence Network (NMHIN) can be categorised into three sections and is as follows:</p><p> </p><p>1) Capacity – NMHIN increased capacity in 2015/16 through NHS England investment. This enabled recruitment to work on additional/enhanced programmes on Crisis Care and Perinatal Mental Health and the first stage of a mental health Joint Strategic Needs Assessment toolkit. NHS England have committed to continue investment in 2016/17.</p><p>2) Products - the NMHIN has enhanced mental health intelligence capability through the development of;</p><p>a) Profiling tools (i.e. new tool on Suicide Prevention; expanded tool on Severe Mental Illness and Common Mental Health Disorders; updated tools which include; Children and Young People’s Mental Health and Wellbeing) and,</p><p>b) ntelligence products (i.e. Measuring Mental Wellbeing in Children and Young People).</p><p>3) Dissemination – the NMHIN has enabled more people to use mental health intelligence through a communication and training programme that provides: routine updates on products and work programmes, training sessions on using products, online access to video and document guides, and a programme of presentations that promote products and encourages use. The Public Health England Local Intelligence Service support this programme and increasingly a range of partners request sessions.</p>

answer text <p>Public Health England (PHE) has worked with the Faculty of Public Health (FPH), Royal Society for Public Health and Royal College of Psychiatrists in developing and implementing the public mental health leadership and workforce development framework. More recently the Royal College of Nursing, Chartered Institute for Environmental Health (CIEH), Association of Directors of Public Health (ADPH) and Institute for Health Visitors have endorsed the framework and signed up to its Call to Action. Mental health has been made a presidential priority for the FPH, CIEH and ADPH. PHE has provided information and guidance to inform the recent FPH curricula review, the Public Health Skills and Knowledge Framework review and the new Mental Health Skills and Knowledge Framework. PHE has also presented the framework to the United Kingdom People in Public Health Board, Skills for Care Mental Health Board, Health Education England Mental Health Advisory Board and the forthcoming Health Education England Public Health Advisory Board.</p>

answer text <p>Since publication, Public Health England (PHE) has worked with national partners and with local authorities to co-ordinate implementation of the public mental health leadership and workforce development (PMHLWD) framework. Twenty national partners have signed the Call to Action to implement the framework.</p><p>PHE is working closely with Health Education England (HEE) to incorporate the PMHLWD framework into the forthcoming Mental Health Skills and Knowledge Framework (commissioned by the Department). It has been cited in the Mental Health Task Force report and PHE will continue to work with HEE to implement the PMHLWD framework’s priorities via the Task Force and Future in Mind working groups.</p><p>PHE has incorporated the PMHLWD framework into the revised Public Health Skills and Knowledge Framework, the review of the future workforce and its wider workforce programme. This has included integrating mental health and wellbeing into the Making Every Contact Count programme.</p><p>PHE has supported delivery via local public mental health networks joint commissioning of training, collating examples of training programmes, developing e-learning modules and delivering local workshops and presentations.</p>

answer text <p>There is no general data collection collected by the health services on the numbers of children and adults who have experienced sexual abuse or physical abuse.</p><p><strong> </strong></p><p>Specific data collections for the purposes of prevention and targeting support are published by the Health and Social Care Information Centre.</p><p> </p><p>The Safeguarding Adults Collection reported annually by the Health and Social Care Information Centre focuses on adults at risk of abuse for whom safeguarding concerns are raised with local councils which instigate investigations under the local safeguarding procedures.
