answer text <p>The Department has not had any discussions with local authorities in Greater Manchester on this issue. It is for local authorities to decide how best to commission and deliver care services in their area.</p><p> </p><p> </p><p> </p><p>However, the Department recently held discussions with the Local Government Association and the Association of Directors of Adult Social Services to agree how best to allocate additional funding to the worst affected local authorities in England to help address delayed discharges from hospitals to the social care system.</p><p> </p><p> </p><p> </p><p>As a result Manchester City Council and Trafford Metropolitan Borough Council have each received a one-off grant of £325,000 in 2014/15 to be spent for these purposes.</p><p> </p> more like this

answer text <p>Through the Mandate we have asked NHS England to make our health service among the best in Europe at supporting people with long term conditions (LTCs) to live healthily and independently, with much better control over the care they receive.</p><p> </p><p> </p><p> </p><p>NHS England and its partners are working towards the implementation of the House of Care model, which is designed to support the delivery of person-centred, coordinated care. This enables individuals to make informed decisions about their treatment and empowers them to self-manage their LTCs in partnership with health and care professionals.</p><p> </p><p> </p><p> </p><p>NHS England has published a House of Care Toolkit that brings together national guidance, published evidence, local case studies as well as information for patients and their carers. It includes information on what tools and resources are required to achieve person-centred coordinated care and how these can be effectively commissioned. answer text <p>NHS England’s National Clinical Director for musculoskeletal (MSK) conditions, Peter Kay, is working in partnership with the Arthritis and Musculoskeletal Alliance, to develop new MSK clinical networks across England.</p><p> </p><p> </p><p> </p><p>The project to date has been very successful so far in capturing examples of best practice in MSK care across England, bringing together health professionals and commissioners and building a strong consensus on the way forward for models of care for MSK patients across the entire MSK community. Work is ongoing in this area and will continue to be supported.</p><p> </p> more like this
