answer text <p>Ray James, National Learning Disability Director at NHS England outlined that by March 2019 National Health Service commissioners in England plan to decommission just over 900 beds in mental health hospitals previously used by patients with a learning disability, autism or both.</p><p> </p><p>The Assuring Transformation (AT) dataset provides data on the numbers of people with learning disability and/or autism admitted and discharged from mental health hospitals.</p><p> </p><p>Data is not collected centrally on the number of people with learning disability and/orautism admitted and discharged from all hospitals.</p><p> </p><p>The following tables show admissions from the community and discharges to the community for the last three financial years. Figures reported are for the number of admissions and discharges and not the number of people as this is the format in which the data are collected. Presenting the data in this way also enables discharges and admissions over the three years to be directly compared. Over this three year period some people may have been admitted and discharged more than once. All data are taken from the AT data set as at 31 March 2018. answer text <p>The following table shows the number of patients recorded in Assuring Transformation (AT) as discharged to the community from an inpatient setting following a Care and Treatment Review (CTR) which recommended discharge, for the last two financial years, 1 April 2016 to 31 March 2018. Information about CTRs has only been collected in the AT dataset since January 2016.</p><p> </p><p>Figures reported are for the number of people discharged at any point during the year; some patients may have been admitted and discharged more than once within the year. answer text <p>Since 2012 the National Survey of Bereaved People (VOICES) survey has provided valuable insight into the quality of care delivered to people in the last three months of their lives, highlighting variations in the quality of care delivered in different areas of the country and to different groups of patients. Following publication of the last set of survey results in June 2016, NHS England held a consultation on the future of the VOICES survey to seek views on the approach and relevance of the survey to ensure that it remained fit for purpose. Whilst the response showed that the VOICES survey remained a valuable tool, key amongst its findings were that the majority of respondents indicated that the VOICES survey would be more helpful if the sample size were made large enough to report at a local commissioner level.</p><p>Following this, work was undertaken to revise the survey and consider approaches to a larger sample size and then put in place arrangements to re-commission the VOICES survey. NHS England has been involved in discussions with the Office for National Statistics, which collects the death registration data used to identify survey recipients, about arrangements for access to the data for the new survey. Changes to the safeguarding arrangements on data-sharing, designed to ensure any concerns about care raised via the survey can be appropriately investigated, have resulted in delays to commencing the new VOICES survey. answer text <p>The Better Care Fund (BCF) is a universal policy for local authorities and clinical commissioning groups to pool budgets for the purposes of integrated care. Local areas develop BCF plans according to their local needs (including mental health), as outlined in the Integration and Better Care Fund Planning Requirements for 2017-19*. Plans are agreed locally by Health and Wellbeing Boards. Data is not collected centrally on how much BCF funding is used to address mental health needs. answer text <p>The Government’s Challenge on Dementia and associated implementation plan describes a programme of action to transform care and experience for people living with dementia, their carers and families by 2020. We are committed to implementing the Challenge in full.</p><p> </p><p>We are currently undertaking a Review of the Challenge on Dementia implementation plan which will reflect on what has been achieved so far and what more needs to be done to meet the commitment. The results will inform the next stage of the Challenge, and establish what additional actions can be taken to ensure the commitments are met by 2020 as planned.</p><p> </p><p>We have acknowledged that an ageing society means that we need to reach a longer-term sustainable settlement for social care. This is why the Government has committed to publishing a Green Paper by summer 2018 setting out its proposals for reform.</p><p> </p><p>The steps that we are already taking will address the issues raised in the reports to ensure that people with dementia receive high quality care and support from health and social services at all times.</p>
