answer	answer is ministerial correction date of answer 19/06/2018 answer text <p>Medium Secure Unit (MSU) Learning Disability (LD) services continue to be provided at the Mersey Care NHS Foundation Trust Whalley site - there are 40 beds available and currently occupancy is running at 100%.</p><p> </p><p>A new 40 bed MSU LD service has now been approved and building has commenced at the Maghull Health Park. The policy should be included in the trust’s Sustainable Development Management Plan (SDMP). The publication of a trust’s SDMP is a requirement under Service Condition 18 of the NHS Standard Contract. answer	answer is ministerial correction date of answer 20/06/2018 answer text <p>It is the responsibility of local providers and commissioners of NHS services to make the reasonable adjustments required by the Equality Act 2010 to ensure that disabled people, including those with sight and hearing impairments, are not placed at a substantial disadvantage compared to non-disabled people.</p><p> </p><p>Additionally, under the NHS Constitution, National Health Service organisations are required to provide high quality comprehensive services, based on clinical need, which do not discriminate between patients on the basis of disability, including sight and hearing impairments.</p><p> </p><p>In order to reduce the unacceptable variation in the provision of accessible information and communication support to disabled people, all organisations that provide NHS care or publicly-funded adult social care have been legally required to follow the Accessible Information Standard since 1 August 2016. This ensures that people with a disability, impairment or sensory loss are provided with information they can easily read or understand, with support, so they can communicate effectively with services. answer	answer is ministerial correction date of answer 18/06/2018 answer text <p>The use of the World Health Organization (WHO) safety checklist for surgical interventions is universally acknowledged to be best practice and is well supported by the relevant Royal Colleges and professional associations. Trusts are expected to track performance against elements and stages of the checklist. There are three stages: ‘sign in’, ‘time out’ and ‘sign out’.</p><p> </p><p>Huddles correspond to the ‘time out’ stage of checklist completion. answer	answer is ministerial correction date of answer 20/06/2018 answer text <p>The Department has not made any specific assessment of the value of the process of safeguarding adult reviews into the deaths of people sleeping rough.</p><p> </p><p>The Care Act statutory guidance states that Safeguarding Adult Boards are required to order a Safeguarding Adult Review if an adult dies in their area and there is concern that partner agencies could have done more to prevent the death and protect the adult.
