answer	answer is ministerial correction date of answer 28/03/2019 answer text <p>Her Majesty’s Prison and Probation Service conduct regular audits of prisons which include examining whether staff employed on adjudication duties are properly trained and competent to carry out these procedures. It also reviews whether or not governors are reviewing the conduct of adjudications to ensure that they are fair, lawful and just, that punishments are normally within locally published guidelines and proportionate, and that no prisoner is charged or punished for any other reason than their disciplinary behaviour.</p><p> </p><p>Any deficiencies feature in the audit report and contribute to the overall audit rating for the establishment. answer	answer is ministerial correction date of answer 28/03/2019 answer text <p>Figures showing the number of assault incidents at each prison in England and Wales between October 2017 and September 2018 are in the attached table.</p><p> </p><p>We have recruited over 4,700 more prison officers since October 2016, and we now have the greatest number in post since early 2012. The Challenge, Support and Intervention Plan case management process for prisoners at risk of violence has been mandated for all prisons and will be fully in place by the end of April 2019. We are improving perimeter security and using other equipment such as body scanners to address the supply of drugs. answer	answer is ministerial correction date of answer 28/03/2019 answer text <p>Community Rehabilitation Company (CRC) contracts require each CRC to maintain a sufficient level of staff and ensure that its workforce is competent and adequately trained. CRCs have the authority to manage staff numbers as per their business and operating models. Robust assurance of performance is provided through operational contract management backed up by regular Operational Assurance audits, and Internal Audit activity as required. There is regular monitoring of service delivery, including a focus on validating published performance management information. CRC performance information showing services levels against a range of measures is released quarterly as part of the Community Performance Quarterly Management Information.</p><p> </p><p>We take seriously concerns raised by Her Majesty’s Inspectorate of Probation (HMIP) and others over staffing levels, particularly for resettlement work with prisoners nearing the end of their custodial term. The contract changes we agreed with CRCs last year include £22m per year in additional funding for Through The Gate services, which will mean approximately 500 more staff working with offenders in prison after April 2019. We are currently proposing further changes to probation after the existing CRC contracts come to an end in 2020. answer	answer is ministerial correction date of answer 27/03/2019 answer text <p>Data on offender learning participation, and learner characteristics, is published by the Department for Education.</p><p> </p><p>Data on learning difficulties and/or disabilities amongst those prisoners who engage in prison education is at: <a href="https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/765596/OLASS_participation_demographic_tool.xls" target="_blank">OLASS: participation and achievement by equality and diversity &amp; English and maths level: 2010/2011 to 2017/18</a>. Self-declared data on learning difficulties and/or disabilities gathered as part of the assessment of prisoners’ levels of maths and English on reception is at: <a href="https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/765597/OLASS_English_and_maths_assessments_by_Ethnicity_and_disability_201415_to_201718.xlsx" target="_blank">OLASS English and maths assessments by ethnicity and learners with learning difficulties or disabilities: participation 2014/2015 to 2017/2018</a>. The data consistently show that around one third of prisoners have a learning difficulty and/or disability.</p><p> </p><p>In April 2016, the Department for Education published a breakdown of the type of learning difficulty and type of learning disability amongst prisoner learners for academic years 2011/12 to 2014/15. That data is at: <a href="https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/517212/offender-learning-lldd-v2.xls" target="_blank">Offender Learning breakdown by disability and learning difficulty 2011/12 to 2014/15</a>.</p><p> </p><p>From 1 April, prison governors will arrange for all new prisoners to be screened for learning difficulties and/or disabilities on reception. Since publication of The Lammy Review (into the treatment of, and outcomes for, Black, Asian and Minority Ethnic individuals in the Criminal Justice System) we have published brand new ethnicity data or analysis in 16 releases.</p><p>Currently HM Prison and Probation Service do collect data on religious beliefs via prisoner records and for offenders in the community. The Offender Equality Annual Report(1) provides religious belief breakdown data on prison population, gender, Incentives and Earned Privileges status, those on Mother &amp; Baby units and adjudications charged and proven.
