answer text <p>The Department does not stipulate what surgical techniques should be used. In the absence of positive National Institute for Health and Care Excellence (NICE) technology appraisal guidance on a particular treatment, it is for National Health Service commissioners to make funding decisions based on an assessment of the available evidence and on the basis of an individual patient’s clinical circumstances.</p><p> </p><p>The commissioning of cataract surgery is the responsibility of clinical commissioning groups, and decisions on the use of particular techniques are made locally on the basis of evidence to inform commissioning decisions.</p><p> </p><p>The National Institute for Health Research is funding a clinical trial comparing standard cataract surgery with femtosecond laser assisted cataract surgery. The report is expected to be published in 2018.</p><p> </p><p>In light of concerns about lengthy waits for treatment and unacceptable variations in care, the Secretary of State has asked NICE to bring forward its guidance on cataracts from 2018 to 2017. This will provide NHS commissioners with evidence based guidance from NICE and help to ensure patients have access to the most effective treatment as early as possible.</p>

answer text <p>The proposals in the consultation that relate to reducing the outpatient follow-up price to increase the first attendance are aimed at incentivising the reduction of clinically inappropriate follow-up appointments. It is recognised that not all services are run in the same way and this is why not all follow-up prices were subject to a 30% reduction. Based on an assessment of the specialties, a 10% or 20% percent reduction was applied where appropriate. The services in the 30% category are in the main, surgical, where follow-up rates vary considerably and some may be inappropriately high. In response to sector feedback NHS Improvement (NHSI) made changes to the categorisations before publishing the consultation. If there are pathways within specialities that may be adversely affected, NHSI will consider further guidance to providers and commissioners on the basis of responses to the consultation which closed on 6 December.</p><p> </p><p>If providers and commissioners believe that the funding as described in the national tariff is not sufficient to support a service, and the commissioner does not have adequate community provision in place, then the national tariff allows providers and commissioners to agree variations that will allow for the correct provision of care. This could involve agreeing changes to prices, or the way that care is funded. NHSI and NHS England will look at new patient-centred ways of contracting healthcare and are working closely with providers and commissioners to ensure that they allow for care to be delivered in a more joined up way between healthcare providers.</p>

answer text <p>The United Kingdom is a world leader in tobacco control and is well advanced in the implementation of the Framework Convention on Tobacco Control (FCTC). In the Autumn Statement, the Government announced legislation to require the licensing of tobacco manufacturing equipment which also takes the UK a further step forward towards ratifying the FCTC’s Illicit Trade Protocol.</p><p> </p><p>In November, the seventh Conference of the Parties considered a report on e-cigarettes by the World Health Organisation and concluded that further monitoring of market developments, emerging research, development of emission testing methodology and regulatory response is required, but that Parties should be invited to regulate these products “as appropriate to their national laws and public health objectives.” The Government has already introduced a regulatory framework for these products in the Tobacco and Related Products Regulations 2016 and The Nicotine Inhaling Products (Age of Sale and Proxy Purchasing) Regulations 2015. The implementation and effect of these regulations will be monitored and the Government will examine all opportunities to maximise the health benefits of these products for smokers whilst minimising their uptake by children and non-smokers.</p><p> </p><p>The Government will also shortly publish a new Plan which will set out a framework for future action on tobacco control.</p>
