Friend the Secretary of State wrote to all Chairs of NHS organisations on 2 June 2015 setting out a range of measures to ensure executive pay is proportionate and justifiable. He asked the Chairs urgently to review their policies on executive pay; to seek the views of Ministers before making any executive appointment paid more than £142,500 per year; to advise him of all current executive pay more than £142,500 and to provide a justification; to ensure that the HM Treasury guidance on off-payroll executive appointments is followed rigorously. He also announced his intention of introducing a limit on the rates payable to off-payroll interim executives; of introducing a national framework for deciding executive pay throughout the NHS; of clamping down on executives who retire and then return to NHS employment so that they do not gain financially from this; and set out his expectation that new redundancy terms should apply to all executive staff. However, NHS England regional teams are able to provide advice and/or support in order to assist the incident management and investigation process, particularly in complex situations where multiple organisations (including NHS providers and commissioning organisations) are involved. In such circumstances NHS England may, for example, facilitate discussions relating to who is the most appropriate organisation to take responsibility for co-ordinating the investigation process. Or, where no one provider or commissioning organisation is best placed to assume this responsibility, NHS England may agree to lead this process.</p><p> </p><p> </p><p> </p><p>In relation to the Lancashire Care NHS Foundation Trust, enquires have been made with the Trust and local clinical commissioning group (CCG) and neither are able to find any intelligence in relation to a request for a joint investigation.</p><p> </p><p> </p><p> </p><p>The only incidents that would potentially be investigated by NHS England are those identified in the Serious Incident Framework i.e. independent investigations and NHS England’s role in the Serious Incident Framework is that of a commissioner for directly commissioned services to maintain oversight and surveillance of serious incident management and assures that CCGs have systems in place to manage investigations appropriately.</p><p> </p><p> </p><p> </p><p>The revised Serious Incident Framework (published March 2015) provides details relating to the roles and responsibilities of NHS England with regards to management serious incidents. 	answer	answer is ministerial correction date of answer 14/10/2015 answer text <p>Train and station operators are required by their operating licences to establish and comply with a disabled people's protection policy (DPPP). This must be approved by the Office of Rail and Road (ORR), the independent regulator of the railways, which took over responsibility for approving and monitoring DPPPs from the Department in October 2013.</p><p>A DPPP sets out the arrangements and assistance that an operator will provide to protect the interests of disabled people using its services and to facilitate such use. Network Rail has a DPPP setting out the assistance it will provide at London Euston and at the other 17 stations it manages.</p><p>If there is evidence that Network Rail is not achieving good outcomes for passengers this should be referred to Network Rail so they can put it right, or to ORR if it is not resolved. ORR can take a number of steps where it finds evidence that there is a problem, including, ultimately, following its enforcement policy where other means have failed.</p><p>I have contacted Network Rail on this point who have informed me that they offer a free mobility assistance service to all passengers that feel they need an extra hand during their travel to and from Euston station. This includes help boarding and alighting trains, assistance with onward journeys on other forms of transport, access to the station toilets and access to the first class lounge. This service does not include assistance to and from the shops on the concourse. NICE makes every effort to ensure that appropriate stakeholders, including clinical experts, are able to engage with the development of its guidance.</p><p> </p><p> </p><p> </p><p>As part of NICE’s technology appraisal process, clinical specialists, commissioning experts and patient experts are invited by the Appraisal Committee to provide written evidence and attend the Committee meeting to help in the Committee's discussion of the technology being appraised.</p><p> </p><p> </p><p> </p><p>NHS England is responsible for administering the Cancer Drugs Fund (CDF) and decisions on which treatments are included on the national CDF list are made by the CDF clinical panel.
