<p>In January 2016, the Department published a joint declaration on Dementia Post-Diagnostic
Care and Support signed by leaders across health, social care, local government and
voluntary sector organisations.</p><p> </p><p>The purpose of the Declaration is to
ensure that the views of people living with the effects of dementia and their families
and carers are taken fully into account when determining the nature of post-diagnostic
services offered.</p><p> </p><p>The Care Act 2014 further requires that when an individual
is assessed as having eligible care and support needs, including those with dementia,
they will have a personalised care plan that details their care and support needs,
and how these will be met. The adult, their carer, and any other person the adult
requests, must be involved in the development of the care plan, and agree with the
content of the plan.</p>
