However, the Government has legislated to establish the Better Care Fund from 2015-16 to provide better integrated care. One of the conditions of the Better Care Fund is to improve the delivery of health and social care by preventing people reaching crisis point, and to reduce the quantity of non-elective admissions to acute care, which may serve to ease the pressures on A&amp;E departments.</p><p> </p> more like this

star this property answer text <p>The treatment of pernicious anaemia, the result of a vitamin B12 (cobalamin) deficiency, is well established and reported in the British Committee for Standards in Haematology (BCSH) document, <em>Guidelines for the diagnosis and treatment of Cobalamin and Folate disorders</em>, which was updated in May 2014. The BCSH operates independently of the Department and NHS England.</p><p> </p><p> </p><p> </p><p>Current clinical practice within the United Kingdom is to treat pernicious anaemia with Vitamin B12 intramuscular injections. The BCSH produces evidence based guidelines for both clinical and laboratory haematologists on the diagnosis and treatment of haematological disease, drawing on the advice of expert consultants and clinical scientists practicing in the United Kingdom. The guideline states that standard maintenance therapy for patients presenting without neurological symptoms is 1,000 microgrammes every three months. Those with initial neurological symptoms should receive 1,000 microgrammes every two months. Recovery data will be included in the children and adolescent mental health services Minimum Data Set which is due to flow to the Health and Social Care Information Centre from January 2016.</p><p> </p><p> </p><p> </p><p>The National Institute for Health and Care Excellence (NICE) has previously reviewed the results of therapies in detail (Clinical Practice Guideline No. 72), with the conclusion that both drug interventions and psychological interventions carry benefit over no treatment, and should be used according to individual need. There has been recent research on the value of the two approaches; and NICE is at present reviewing the research and how to update its recommendations.</p><p> </p> more like this

Officials have also discussed the issue of apprenticeships for people with autism with the Departments for Work and Pensions and Business, Innovation &amp; Skills and these discussions will continue. A number of the recently announced Autism Innovation Fund projects focus on employment and apprenticeships and their outcomes will also help work in this area. star this property answer text <p>It is the responsibility of local employers to ensure that they have the right numbers of staff, with the right skills available to provide a high quality service to their patients. Locally there should be specialist training for those in roles that have a direct impact on and make decisions about people with autism.</p><p> </p><p> </p><p> </p><p>This will be re-enforced in new statutory guidance on autism for local authorities and the National Health Service which is to be issued shortly. In addition clinical staff such as psychiatrists and clinical psychologists should be undertaking their own clinical professional development. In support of this Health Education England will provide support in raising awareness of autism. In Hull, the clinical commissioning group is working with local providers, particularly Humber NHS Foundation Trust, to recruit additional staff for its Autism Diagnosis Team.</p><p> </p> more like this

star this property answer text <p>The Care Quality Commission’s inspection of hospitals will include assessment of whether or not children’s needs are being assessed and care and treatment delivered in line with legislation, standards and evidence-based guidance. The inspection framework for National Health Service acute hospitals for services for children and young people was last updated in October 2014. Hospital trusts are able to measure effectiveness through, for example, the Friends and Family test, the NHS National Children's Inpatient and Day Case Survey and other means of engaging with children and their families.</p><p> </p><p> </p><p> </p><p>The guidance which the NHS should follow in ensuring the quality of care provided for children with autism in hospital is issued by the National Institute for Health and Care Excellence, chiefly the quality standard [QS51] on Autism, published in January 2014 and guidelines, autism: the management and support of children and young people on the autism spectrum (August 2013), which is available at <a href="https://www.nice.org.uk/guidance/cg170" target="_blank">https://www.nice.org.uk/guidance/cg170</a></p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p>

star this property answer text <p>NHS England has advised that the provision of a neuromuscular care co-ordinator within the multidisciplinary team was a key requirement of the specialised commissioning, service specification compliance exercise.</p><p> </p><p> </p><p> </p><p>At the time of the initial assessment in 2013, the service at Barts Health NHS Trust was rated compliant in this area, which included the provision of a neuromuscular care co-ordinator.</p><p> </p><p> </p><p> </p><p>The NHS Trust Development Authority has advised that Barts Health NHS Trust has agreed to prepare a business case to explore the possibility of creating a trust funded neuromuscular care coordinator post and will explore the possibility of obtaining charity funding for this post.</p><p> </p> more like this

star this property answer text <p>The Department has made no estimate of the recovery or mortality rates of cancer patients or patients who have had a heart attack, who also experience depression.</p><p> </p><p> </p><p> </p><p>The National Institute for Health and Care Excellence advises that treating depression in people with a chronic physical health problem has the potential to increase their quality of life and life expectancy. Depression often has a remitting and relapsing course, and symptoms may persist between episodes. Where possible, the key goal of an intervention for depression should be complete relief of symptoms (recovery) – this is associated with better functioning and a lower likelihood of relapse than lesser degrees of response, as well as potentially better physical health outcomes.</p><p> </p><p> </p><p> </p><p>Over £400 million is being invested over the spending review period to make a choice of psychological therapies available for those who need them in all parts of England.</p><p> </p><p> </p><p> </p><p>We are also investing in improving provision for children and young people, older people and carers, people from black minority and ethnic groups, people with severe mental illness and people with long-term physical health problems.</p><p> </p><p> </p><p> </p><p>The Department’s 2014-15 Mandate to NHS England makes clear that ‘everyone who needs it should have timely access to evidence based services’, this includes services for people with depression. The Mandate sets a clear objective for NHS England to deliver the key objectives of the Improving Access to Psychological Therapies programme – providing access to therapies for 15% of those eligible (around 900,000 people), with a recovery rate of 50%.</p><p> </p><p> </p><p> </p><p>NHS England is also currently carrying out a pilot aimed at improving access to psychological therapies for people with long term conditions and/or medically unexplained symptoms including diabetes, chronic obstructive pulmonary disease, stroke and other illnesses/disorders.</p><p> </p><p> </p><p> </p><p>The refreshed Mandate to NHS England for 2015-16 makes clear that by March 2016 we expect NHS England to have made further measurable progress towards achieving true parity of esteem where everyone who needs it has timely access to evidenced based services that are better integrated with physical health services.</p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p><p> </p>

star this property answer text <p>The Care Quality Commission (CQC) is the independent regulator of health and adult social care providers in England. Under the Health and Social Care Act 2008 (the 2008 Act) all providers of regulated activities, including National Health Service and independent providers, have to register with the CQC and meet a set of requirements of safety and quality.</p><p> </p><p> </p><p> </p><p>The 2008 Act, together with the CQC (Registration) Regulations 2009 require that all providers must have in place one or more registered managers for its regulated activities. The regulations set out some exceptions to this (for example if the provider is a health service body, or a lone individual who meets certain criteria) but in practice, the vast majority of registered providers in the social care sector must have a registered manager.</p><p> </p><p> </p><p> </p><p>Registered managers have legal responsibilities in relation to their position. The person appointed as registered manager should be in day-to-day charge of carrying on the regulated activity or activities they apply to be registered for. The regulations do not specify that the registered manager must be on the premises at all times, but in all cases, the registered manager must be able to demonstrate how they will manage the day-to-day running of the regulated activities at each of their locations.</p><p> </p><p> </p><p> </p><p>The CQC has supplied the following information about the registered manager regulations:</p><p> </p><p><strong> </strong></p><p> </p><p>As of 5 February 2015 there are 16,426 locations which provide the regulated activity of accommodation for people who require nursing or personal care. The provider is required to have a registered manager in place for each of these locations. 14,758 locations have a registered manager in place and 1,668 locations do not.</p><p> </p><p> </p><p> </p><p>The CQC inspectors ensure that where a manager is not in place the provider complies with <em>Regulation 14 Care Quality Commission (Registration) Regulations 2009, Notice of absence</em><strong>, </strong>to ensure the safe management of the home while a manager is absence or in the process of being appointed. The expectation is that providers will take timely and strenuous action to appoint a manager who is suitably competent and qualified to apply for and secure registration. If providers fail to do so, in addition to enforcement action which can include the issue of a Fixed Penalty Notice or prosecution CQC will limit the rating when judging the “Well led” question as part of CQC’s new approach to inspection and ratings.</p><p> </p><p> </p><p> </p><p>The requirements state that registered managers must have the necessary qualifications, skills and experience to manage the carrying on of the regulated activity. The CQC assesses against the requirements in Regulation 7(2) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2010 when it registers providers and managers (by asking about qualification, induction and ongoing training programmes, for example), as well as monitoring continuing compliance with this requirement through inspection. In CQC’s Key Lines of Enquiry, CQC inspectors are prompted to look at how providers are making sure that staffing levels have the right mix of skills, competencies, qualifications, experience and knowledge, to meet people’s individual needs, to determine whether or not a care home provider is delivering safe care.</p><p> </p>

A finished admission episode (FAE) is the first period of admitted patient care under one consultant within one healthcare provider. FAEs are counted against the year or month in which the admission episode finishes. Admissions do not represent the number of patients, as a person may have more than one admission within the period.</p><p> </p><p>3. Changes to the figures over time need to be interpreted in the context of improvements in data quality and coverage (particularly in earlier years), improvements in coverage of independent sector activity (particularly from 2006-07) and changes in NHS practice.</p><p> </p><p>4. FAEs recorded as &quot;The usual place of residence, including no fixed abode&quot; may contain a number of episodes where patients may have resided in care homes, subsequently designating it as their usual place of residence. Consequently, these episodes were not recorded as admissions from a care home. The number of times this may have occurred is unknown.</p><p> </p>
