The Immigration White Paper, ‘The UK’s future skills-based immigration system’, published in December 2018, sets out the foundation for a single immigration system, where it is workers’ skills that matter, not where they come from. All UK registered doctors are expected to meet the professional standards set out in the GMC's guidance ‘Good Medical Practice’. 	answer	answer is ministerial correction date of answer 15/07/2019 answer text <p>As in preparation for 29 March and 12 April, our ‘no deal’ contingency plans for 31 October cover the National Health Service, social care and the independent sector and covers all medicines, medical devices and clinical consumables, supplies for clinical trials, vaccines and countermeasures, and organs and tissues for transplants.</p><p>Specialist nutritional products including prescription foods and synthetic protein supplements are an important part of patient care throughout the health and social care sector. In recognition of this we have made this a specific area of focus within our preparedness arrangements.</p><p>The Department has continued to work closely with the devolved administrations, industry trade bodies including the British Specialist Nutrition Association and suppliers, the NHS and other key stakeholders. Prior to 29 March, the Department analysed the supply chains of 12,300 medicines, close to half a million product lines of medical devices and clinical consumables, vaccines used in national and local programmes, and essential non-clinical goods on which the health and care system relies. These analyses and assessments have been updated where necessary and will continue to be refined while a ‘no deal’ European Union exit remains a possibility. This work has informed our plans.</p><p>We are asking suppliers to analyse their supply chains and to make alternative plans if they anticipate disruption, including rerouting to alternative ports or using airfreight where necessary. The first 630 places were taken up in September 2018 and the remaining additional places will have been made available by universities by 2020/21. By 2020, five new medical schools will have opened in England to help deliver the expansion.</p><p>The number of medical specialty training places that are available each year is set by Health Education England and based on their assessment of service gaps and predicted workforce needs.</p><p>However, it is the responsibility of National Health Service trusts to have staffing arrangements in place that deliver safe and effective care. The approach NHS England must take in setting allocations is outlined in the mandate from the Department which says:</p><p><em> </em></p><p>“The Government expects the principle of ensuring equal access for equal need to be at the heart of NHS England’s approach to allocating budgets.”</p><p> </p><p>The approach is also informed by NHS England’s duty to reduce inequalities to accessing services and the outcomes of care.</p><p> </p><p>These two aims are reflected in the target formula, which produces a target allocation or ‘fair share’ for each area, based on a complex assessment of factors such as demography, morbidity, deprivation, and the unavoidable cost of providing services in different areas. 	answer	answer is ministerial correction date of answer 22/07/2019 answer text <p>In the development of each national tariff, NHS England and NHS Improvement undertakes an overall assessment of the impact of proposals. While this provides an overall assessment of impact, detailed feedback on proposed prices for individual HRGs (units of healthcare) is sought from clinical Expert Working Groups (EWGs). This is to ensure that the published tariff supports clinical practice.</p><p>For the 2017/19 tariff (set for two years), the prices for BZ91 were increased following EWG feedback. However, for the 2019/20 tariff the EWG advised that increasing the prices was no longer appropriate. The EWG felt that there had been a significant change in clinical practice since the 2017/19 prices were set, with a shift to performing minimally invasive glaucoma surgery (MIGS), which are significantly cheaper, rather than open/tube glaucoma procedures. Based on diagnosis and procedure codes entered into patient records, MIGS activity is assigned to the HRG BZ91. However, the current codes do not allow differentiation between MIGS and non-MIGS activity. This means that BZ91 covers procedures of different levels of resource use.</p><p>Reference costs are collected and published annually and the latest published data is for the financial year 2017-18.
