star this property answer text <p>1.3 million of the highest risk patients whose immune systems mean they are at higher risk of progression to severe disease, hospital admission or death can receive antivirals directly in England via COVID Medicines Delivery Units.</p><p>Elderly people are eligible for the antiviral molnupiravir through the PANORAMIC study run by the University of Oxford. This study is open to individuals in the United Kingdom who have received a positive polymerase chain reaction test, are aged 50 years old and over or are aged 18 to 49 years old with an underlying medical condition which can increase the risk of developing severe COVID-19. We are awaiting outcomes from the PANORAMIC study before confirming plans to expand the eligible cohort.</p> more like this

star this property answer text <p>A range of treatment options are available to non-hospitalised patients at higher risk from COVID-19 to reduce severe disease, hospitalisation and death. These treatments are available through COVID Medicines Delivery Units and include the oral antivirals nirmatrelvir+ritonavir (Paxlovid), molnupiravir and the infusion antiviral remdesivir.</p><p>Evidence from clinical trials suggests these treatments can reduce the risk of hospitalisation. For molnupiravir, interim results found a 50% reduction in the relative risk of hospitalisation or death compared to placebo. However, updated results indicate molnupiravir reduces the relative risk of hospitalisation or death by 30% compared to placebo. Pfizer’s final analysis on the clinical trials of nirmatrelvir+ritonavir showed an 88% reduction in hospitalisation or death compared to a placebo within five days of symptom onset. No assessment of how these treatments have impacted rates of hospitalisation since December has been made.</p><p>Both molnupiravir and nirmatrelvir+ritonavir are being trialled as part of the PANORAMIC national study. Results for molnupiravir are expected in summer 2022. Recruitment for the nirmatrelvir + ritonavir opened on 11 April 2022. The study will collect further data on how these treatments work in a United Kingdom context where the majority of the population is vaccinated. It will also provide baseline information on how antivirals could be used for best clinical effect in combination with antibodies or other antiviral drugs as they become available. A 2021 trial showed that among non-hospitalised patients at high risk for COVID-19 progression, a three-day course of remdesivir had an acceptable safety profile and resulted in an 87% lower risk of hospitalisation or death than placebo.</p>

star this property answer text <p>Although several potential drugs for COVID-19 are being trialled around the world, few results have been reported yet, and we do not know if any could help save people who are already seriously ill when diagnosed.</p><p>The World Health Organization (WHO) along with several countries in Europe, including the United Kingdom, are coordinating an international trial of the most promising drugs which include the long-used antimalarial drugs chloroquine and hydroxychloroquine, a new antiviral drug called remdesivir and a combination of two HIV drugs called lopinavir and ritonavir. The HIV drugs will also be tested in combination with an antiviral called interferon beta.</p><p>The trial will focus on whether any of these drugs reduce mortality or the time patients are in hospital and whether patients receiving drugs require ventilation or an intensive care unit.</p><p>On 3 April, the Government also announcement that the world’s largest randomised clinical trial of potential coronavirus treatments is well underway in the UK as part of the race to find a treatment.</p><p>Scientific and medical advisers are already developing a new vaccine against the COVID-19, as they initiated vaccine development as soon as the genetic sequence of the novel coronavirus was released.</p>

star this property answer text <p>As of 28 March 2022, the number of patients currently in hospital with COVID-19 is 17,685. A range of treatments are available to these patients, including antivirals. Eligible patients admitted to hospital for reasons not related to COVID-19 and subsequently test positive are able to access nirmatrelvir + ritonavir (Paxlovid), an oral antiviral treatment or remdesivir, an intravenous antiviral. For patients whose immune system means they are at higher risk of COVID-19, the antiviral treatments molnupiravir, nirmatrelvir + ritonavir and remdesivir are available in the community to reduce the risk of hospitalisation and death.</p><p> </p><p>Approximately 120,000 treatments have been provided in England, of which 53,000 were antivirals administered to hospitalised patients. The data requested on the average time for patients receiving antivirals to be cured is not held centrally.</p> more like this

star this property answer text <p>As part of pandemic-specific arrangements, NHS England was asked to utilise its national clinical policy making expertise to lead on the development of United Kingdom-wide clinical access policy drafts enabling access to evidence-based COVID treatments. These draft policies are then considered and approved by the UK Chief Medical Officers ahead of publication, to guide the decisions of treating clinicians, and supporting medicines supply arrangements.</p><p> </p><p>NHS England has been supported in this role by a range of clinical and academic experts, including representatives from the devolved administrations in Northern Ireland, Scotland and Wales, who meet as part of Expert Working Group arrangements. There is not a standing or single expert working group but rather a series of clinical policy-specific expert working groups that have been established and that meet, as required, with clinical expertise specific to the medicine or medicines being considered. For example, an expert working group was established specific to Evusheld as pre-exposure prophylaxis, which met twice.</p><p> </p><p>Through this expert working group approach, clinicians and academics have provided advice which has helped clinical policy makers to better understand the supporting clinical trial information, to identify any important contraindications or drug to drug interactions that need to be considered, and to understand how best to place a potential new medicine option alongside current treatment choices. This has provided NHS England with both clinical subject matter and specialty expertise and a valuable ‘front line’ perspective in developing access policy proposals.</p>
