star this property answer text <p>The comprehensive model for personalised care is made up of six key components:</p><p>- Patient Choice;</p><p>- Patient Activation;</p><p>- Social Prescribing;</p><p>- Shared decision making;</p><p>- Personalised care and support planning; and</p><p>- Personal health budgets.</p><p> </p><p>This is a new model which was developed through the Integrated Personal Commissioning and Empowering People and Communities programmes. The components were tested in 2017/18 across 21 demonstrator sites, who reported 180,000 personalised care interventions for 2017/18. Sites are now rolling out the model and in Q2 2018/19 reported 280,000 personalised care interventions.</p><p> </p><p>The intention, as set in the NHS Long Term Plan, is to roll out beyond demonstrator sites to reach 2.5 million people by 2023/24.</p><p> </p><p>There is a national data collection for personal health budgets. The number of people with a personal health budget was:</p><p>- 15,811 in 2016/17;</p><p>- 28,040 in 2017/18; and</p><p>- 32,341 in 2018/19 (latest figure from Q2).</p>

The Social Care Green Paper remains a priority for this Government, and the Department is working hard to publish the Green Paper at the earliest opportunity.</p><p> </p><p>Given wider events it was not possible to publish the Green Paper by the end of 2018 and, unfortunately, we cannot currently confirm a publication date.</p> more like this

star this property answer text <p>The NHS Long Term Plan, published on 7 January 2019 sets out where extra National Health Service funding will be focused and how services will be designed, including the importance of ensuring speech and language therapy is joined up with children’s mental health services. Work will take place with local areas to design and implement models of care for children and young people that are age appropriate, closer to home and bring together physical and mental health services. These models will support health development by providing holistic care across local authority and NHS services, including primary care, community services, speech and language therapy, school nursing, oral health, acute and specialised services.</p> more like this

star this property answer text <p>Under the Liberty Protection Safeguards, the Approved Mental Capacity Professional criteria will be set out in regulations. Further detail on the role will also be outlined in the Statutory Code of Practice. The Government envisages professionals such as social workers, learning disability nurses, occupational therapists, psychologists and other suitable health and social care professionals could take on the role if they meet the criteria set out in regulations.</p> more like this
