This includes plans to manage patients in line with infection control procedures, and where necessary in highly specialist infectious diseases facilities.A patient with Ebola virus infection would be cared for in the national High Level Isolation Unit at the Royal Free Hospital in London. In the unlikely event of an outbreak involving more than two people, specialist staff at this unit are trained and prepared to look after patients safely in an isolation ward, which would be dedicated for this purpose.</p><p> </p><p>In the UK, the procedures outlined in the Advisory Committee on Dangerous Pathogens guidelines, “The Management of Hazard Group 4 viral haemorrhagic fevers and similar human infectious diseases of high consequence”, are followed for any suspected case.</p><p> </p><p>Public Health England has assessed the risk of importation of Ebola in the UK as very low. It is extremely unlikely that an outbreak of Ebola would occur in <br>the UK even if there was to be an imported case, as there are factors operating <br>in West Africa which would not be seen in the UK. The outbreak in West Africa <br>is driven by person to person spread in the absence of any infection control <br>precautions. Human cases of Ebola virus disease have never yet been exported <br>from an outbreak zone to a European country.<br><br>All UK ports have systems and plans for dealing with issues of public health <br>concern. However, they all require that the public health system is firstly <br>notified of the arrival of someone with a suspected disease of concern. Crew <br>are trained to do this by their respective companies and an example of the type <br>of guidance that they work towards can be found on the International Air <br>Transport Association (the international airline trade body) website at: <br><br><br>https://www.iata.org/whatwedo/safety/health/Documents/health-guidelines-cabin-cr<br>ew-2011.pdf . <br><br>At all UK ports, there is a system for routing reports, produced by the <br>commander of a craft, to a local health protection team who are available on a <br>24/7 basis throughout the year. We keep these plans under constant review.</p><p> </p><p> </p><p> </p><p>These plans have been tested most recently by the successful transfer back to the UK of a healthcare worker who became infected with Ebola when working with patients in Sierra Leone. The patient, a British National, was cared for in the national High Level Isolation Unit at the Royal Free Hospital in London. Additionally, officials from the Department of Health have been in regular contact with their counterparts from the devolved administrations via weekly telecommunications, in which Ebola screening at airports has been discussed. Currently over 1000 NHS staff and 185 staff from Public Health England have put their names forward through UK MED which runs the <em>UK International Emergency Medical Register </em>(UKIEMR).</p><p> </p><p> </p><p> </p><p>The programme comprises a robust selection process, pre-deployment vaccinations and occupational health and psychological screening, training in the UK with further training and mentoring in Sierra Leone, and advice and support on monitoring health on return. The United Kingdom has robust, well-developed and well-tested public health and National Health Service systems for preventing and managing infectious diseases including any imported case of this type of disease, supported by a wide range of experts and specialist units. NHS England has recently received assurance from all NHS acute and ambulance chief executives of their readiness for an Ebola case.</p><p> </p><p> </p><p> </p><p>Screening arrangements are in place at the UK’s main ports of entry for people travelling from the affected regions. Screening involves temperature checks and a questionnaire asking about current health, recent travel history and any contact with Ebola patients. Passengers are required to provide contact details and are given advice on what to do and whom to contact should they develop symptoms.</p><p> </p><p> </p><p> </p><p>In addition to weekly meetings with the Devolved Administrations at official level, I hold regular meetings with the health minister counterparts in the Devolved Administrations on UK Ebola preparedness and response.</p><p> </p><p> </p><p> </p><p>A number of national and local multi-agency exercises have been conducted in recent months to test operational resilience and provide assurance of Ebola preparedness. This included a recent exercise involving all four nations of the UK. The case of the British nurse recently returned from Sierra Leone demonstrated that the systems put in place across the UK to prepare for and respond to a case of Ebola worked well. In the light of that case, Public Health England has reviewed and further strengthened its screening protocols to ensure that anyone from a higher risk group who feels unwell at the time of screening will be reassessed. Advice will also be sought from an infectious diseases specialist and the passenger will be referred for testing if appropriate. The screening centres at Heathrow were viewed first-hand by the Chief Medical Officer and myself on 1 January 2015 and all arrangements, including the revised protocols, were found to be working well. Public Health England (PHE) issued guidance for all medical staff in both primary care and acute trusts on 25 September. In addition three messages have been sent out to the National Health Service, general practitioners and private hospitals through the Chief Medical Officer’s Alerting System (CAS). The most recent of these was on 7 October and it covered assessment of patients for their potential to have Ebola, advice on how to have a test carried out and management of the patient whilst test results are awaited and once the diagnosis is known.</p><p> </p><p> </p><p> </p><p>We are doing considerable work on providing guidance to the medical profession and there was an exercise on 11 October centred around scenarios in which cases of Ebola came to light in the United Kingdom, which heightened public awareness of the issues. In addition posters and leaflets are being made available in ports of entry. The outbreak was declared an international public health emergency by the United Nations in August. The advice from PHE has been extensive, covering issues such as guidance on the management of patients and infection control and prevention for staff.</p><p> </p><p> </p><p> </p><p>The Chief Medical Officer has issued three Central Alerting System alerts on the Ebola outbreak in West Africa, on 2 July, 1 August and 7 October. The alert reminded healthcare providers to remain vigilant for cases imported to the United Kingdom, emphasising the importance of taking a full travel history when assessing relevant patients and information on identifying and managing possible cases of Ebola virus disease and actions for all clinicians to ensure preparedness for the possibility of an imported case in the UK.</p><p> </p><p> </p><p> </p><p>These alerts were cascaded to the entire NHS in England, as well as PHE, PHE Consultants in Communicable Disease Control, PHE Consultants in Health Protection,</p><p> </p><p>Trust Microbiologists, Virologists and Infectious Disease Clinicians, private hospitals across England and Clinical Commissioning Group Leads.
