National Health Service organisations continue to use the 2001 codes in order to support consistency with other data systems within the NHS. NHS organisations can choose to use either the 2001 or the 2011 categories.</p><p> </p><p>Additional codes can be included as appropriate at a local level to reflect the demographic make-up of the local population. 	answer	answer is ministerial correction answer text <p> </p><p>Mogden Sewage Treatment Works has an Environmental Permit issued by the Environment Agency to discharge treated and storm effluent to the Thames Estuary at Isleworth. The discharge of storm effluent is permissible only when the capacity of onsite facilities for treatment and containment are exceeded. This includes a 77,000m³ storm tank.</p><p> </p><p> </p><p> </p><p>The permit does not specify a number of spills per year. It requires the works to provide secondary treatment for a specific flow rate of incoming sewage.</p><p> </p><p> </p><p> </p><p>The current version of the permit came into effect on 31 March 2013. During the subsequent 12 months, Thames Water reported 54 occasions when the works discharged storm sewage to the Thames. 	answer	answer is ministerial correction answer text <p /> <p /> <p>The Parole Board assesses the risk posed by individual prisoners when considering their release or transfer to open conditions, and work that has been completed to reduce these risks, rather than only looking at whether specific offending behaviour programmes (OBPs) have been completed. It is not mandatory for Indeterminate Sentence Prisoners to complete OBPs in order to achieve release. Other work that may help to reduce risk may take the form of accredited OBPs; however, it may also include activities such as education or training, work, one to one sessions with a psychologist and a range of other interventions. Although in some circumstances an OBP may be the preferred option, the Parole Board may consider that an offender has reduced their risk of harm, despite not being able to attend a particular course, because they have been able to undertake other offending behaviour work which has achieved the same outcome.</p><p><strong> </strong></p><p><strong> </strong></p><p>As at 31 December 2013 there were 5,335 prisoners serving an Indeterminate Sentence of Imprisonment for Public Protection, of which 3,561 were beyond the tariff.</p><p>Of these prisoners beyond tariff, 3,160 had completed at least one accredited offending behaviour programme successfully; 415 had attended a programme and not completed it successfully and of these, 62 had yet to complete any programme successfully; and 184 are currently attending a programme and the outcome is not yet known.</p><p> </p><p>The figures have been drawn from administrative IT systems, which, as with any large scale recording system, are subject to possible errors with data entry and processing.</p><p> </p><p>No information is collected on how many prisoners have not been offered an accredited offending behaviour programme and this could only be obtained at disproportionate cost.</p><p> </p><p> </p> question first answered 28/04/2014 12:00:00

	answer	answer is ministerial correction answer text <p>The Organisation for Security and Co-operation in Europe (OSCE)'s Office for Democratic Institutions and Human Rights (ODIHR) launched an Election Observation Mission (EOM) to Ukraine in March. ODIHR's Needs Assessment Mission judged that the EOM should consist of 1000 observers in total. 100 long term observers have been on the ground since late March; while a further 900 short term observers will arrive in the week before polling day on 25 May. These observers will also be joined by parliamentarians from the OSCE Parliamentary Assembly and the Parliamentary Assembly of the Council of Europe amongst others. The ODIHR Mission will be crucial in providing an impartial view of the conduct of these extremely important elections, and the UK is providing over 100 observers to it.</p><p> </p><p>Since March, 7 inspection teams of varying staff numbers from OSCE states have been deployed to Ukraine under the auspices of the Vienna Document to which all OSCE States subscribed, with the aim of building confidence and increasing transparency. This is in response to a Ukrainian request to carry out military inspections and separate from the OSCE Special Monitoring Mission, which has a different mandate. A UK-led Vienna Document team is currently in Ukraine and is working closely with national authorities to carry out their mission. The inspection team is made up of UK armed Forces Personnel and international observers. Following the UK-led mission, Vienna Document inspectors from other participating states will commence similar missions.</p><p> </p> question first answered 13/05/2014 12:00:00
