star this property answer text <p>The National Institute for Health and Care Excellence (NICE) makes recommendations for the National Health Service on whether new licensed medicines, and significant licence extensions for existing medicines, should be routinely funded by the NHS. The NICE is able to recommend most new medicines for use by the NHS, including as a result of commercial agreements negotiated between NHS England and the company, in line with the commercial framework for new medicines.</p><p>The NICE develops its guidance in line with its published health technology evaluations manual, which sets out the circumstances in which its Appraisal Committees are able to apply flexibility in the evaluation of specific treatments. The NICE is, for example, able to apply a modifier in the evaluation of medicines for severe diseases that means that its committees are able to recommend medicines at a higher cost-effectiveness threshold than for less severe diseases. The NICE appraises combination therapies using the same methods that it uses for appraising single drug therapies.</p>

star this property answer text <p>We are aware of supply issues with three pancreatic enzyme replacement therapies: Creon 10,000 gastro-resistant capsules; Creon 25,000 gastro-resistant capsules; and Nutrizym 22 gastro-resistant capsules. We understand that these are due to limited availability of active pharmaceutical ingredients, and manufacturing constraints in producing the volumes required to meet demand. The Department has issued guidance to healthcare professionals regarding treatment of patients while there is a disruption to the supply of these pancreatic enzyme replacement therapies. We are having regular conversations with the suppliers of these products to ask that they expedite deliveries and increase production forecasts, and to confirm that they are taking action to address the root causes of the issues, to ensure continuity of supply. We are also working with specialist importers to source unlicensed imports from abroad.</p><p>Whilst we can’t always prevent supply issues, we have a range of well-established tools and processes to mitigate risks to patients. These include close and regular engagement with suppliers, use of alternative strengths or forms of a medicine to allow patients to remain on the same product, expediting regulatory procedures, sourcing unlicensed imports from abroad, adding products to the restricted exports and hoarding list, use of Serious Shortage Protocols, and issuing National Health Service communications to provide management advice and information on the issue to healthcare professionals, so they can advise and support their patients.</p>

Applications are subject to peer review and judged in open competition, with awards made on the basis of the importance of the topic to patients and health and care services, value for money, and scientific quality. The NIHR has provided funding for research projects associated with the South Asian Biobank through open competition, and welcomes any future applications. In addition, the NIHR provides funding to NIHR research infrastructure through open competition. star this property answer text <p>It is important that the correct start date is captured for patients who transfer from a referral to treatment (RTT) pathway between providers. For patients that move between different areas of England and transfer to a new provider, including children who may relocate out of an area due to housing pressures, there is an established Inter Provider Transfer process which ensures that the patient is transferred with the same RTT clock start date, so that the new provider considers the amount of time the patient has already waited. star this property answer text <p>The Department has been working hard with industry to help resolve supply issues with some attention deficit hyperactivity disorder (ADHD) medicines, which are affecting the United Kingdom and other countries around the world. This has involved asking that suppliers expedite deliveries to boost supplies of these important medicines, and address regulatory issues to ensure continuity of supply. As a result of our ongoing activity and intensive work, some issues have been resolved. Many strengths of lisdexamphetamine and all strengths of atomoxetine capsules and guanfacine are now available, although medicine supply issues remain for some strengths of lisdexamphetamine and methylphenidate. We continue to escalate these issues with manufacturers to ensure that they are taking responsibility for resolving them as quickly as possible, and to drive and monitor progress.</p><p>We monitor and manage medicine supply at a national level so that stocks remain available to meet regional and local demand. The Department does not hold information on stockholdings at a local level.</p>

This works well for the most part, as evidence shows we have amongst the lowest prices in Europe for generic medicines. The system also means we have a relatively resilient supply, as flexibility on pricing allows manufacturers to increase prices where supply on a particular drug is reduced or demand increases, supporting continuity of supply for patients in the United Kingdom.</p><p>Between 2009 and 2017, there was only one supplier of liothyronine, and they were charging a price considered to be excessive and unfair by the Competition and Markets Authority (CMA). Following an investigation which concluded in July 2021, the CMA decided to issue fines totalling £101.4 million against the three undertakings which controlled the supply during this time.</p><p>That decision is currently under appeal before the Court of Appeal, having previously been upheld by the Competition Appeal Tribunal in August 2023. Since 2016, several marketing authorisations have been granted to other suppliers for liothyronine, and its price to the National Health Service has been decreasing as a result of direct competition in the market. The Department has no price comparison information on liothyronine.</p>

star this property answer text <p>While data is not held centrally on diagnosis rates for genetic haemochromatosis, genomic testing for haemochromatosis is available through the NHS Genomic Medicines Service (GMS) for patients in England who show an unexplained iron overload, suggestive of genetic haemochromatosis. Any clinician who suspects haemochromatosis can order this testing via local genomic laboratory hubs.</p><p>Seven NHS GMS Alliances play an important role in supporting the strategic systematic embedding of genomic medicine in end-to-end clinical pathways and clinical specialities. They help to achieve equitable access to standardised end-to-end pathways of care, inclusive of genomic testing, clinical genetics, and genomic counselling services. The alliances also raise awareness among clinicians and the public of the genomic testing available through the National Health Service and drive this embedding across all providers within their geography, from primary and community care to secondary and tertiary care.</p>

star this property answer text <p>The Department works with NHS England to raise awareness of cancers, including for people with increased risk of cancer. In January 2024, NHS England relaunched the Help Us Help You cancer awareness campaign, designed to increase earlier diagnosis by encouraging people to come forward with suspected signs of cancers.</p><p>For individuals with neurofibromatosis type 1 (NF1), who are at increased risk of breast cancer, the National Health Service in England recommends breast screening from the age of 40 years old. It is important that awareness of this is widespread among patients and general practices (GPs). Treatment for NF1 involves regular monitoring, and if a patient develops complex problems, their GP can usually refer them to one of two specialist NHS centres, so that a treatment plan can be drawn up. These centres are at Guy’s and St Thomas’ in London, and at Manchester University Hospital.</p><p>We expect clinicians to keep themselves appraised of developments within conditions, and to refer to the National Institute for Clinical Excellence guidance as part of their clinical decision-making process.</p>

This has involved asking that suppliers expedite deliveries to boost supplies of these important medicines, and addressing regulatory issues to ensure continuity of supply. As a result of our ongoing activity and intensive work, some issues have been resolved. We expect the disruptions to the supply of Concerta XL tablets to be resolved in May 2024.</p><p>The Department held a roundtable with all suppliers of methylphenidate prolonged-release tablet suppliers, including Janssen, in April 2024, to discuss the challenges they are facing, what they are doing, and what needs to be done to address these issues.</p><p>We have worked closely with specialist clinicians during this time to develop management advice for the National Health Service. The guidance includes advice for ADHD service providers and specialists to offer rapid response to primary care teams seeking urgent advice for the management of patients. This includes those known to be at a higher risk of adverse impact because of these shortages.</p>

star this property answer text <p>Research is crucial in tackling cancer, which is why the Department invests £1.3 billion per year in health research, through the National Institute for Health and Care Research (NIHR). NIHR research expenditure for all cancers was £121.8 million in 2022/23, and the NIHR spends more on cancer than any other disease group. <br> <br> The NIHR funded nine research projects on pancreatic cancer since 2018/19, with a committed funding value of £4.3 million. The NIHR continues to welcome funding applications for research into any aspect of human health, including pancreatic cancer. Applications are subject to peer review and judged in open competition. <br> <br> The NIHR also supports delivery of research into pancreatic cancer in the health and care system, funded by research funding partners in the charity and public sectors. Since 2018/19 the NIHR supported 73 clinical research studies through the Clinical Research Network. <br> <br> Additionally, NIHR Biomedical Research Centres (BRC) and NIHR Clinical Research Facilities spent a further £6.5 million between 2018/19 and 2022/23, supporting an annual portfolio of approximately 85 early clinical studies into pancreatic cancer. NIHR BRCs are collaborations between world-leading universities and National Health Service organisations, that bring together academics and clinicians to translate discoveries from basic or discovery science into clinical research. <br> <br> For emotional and financial support, patient feedback is monitored through the National Cancer Patient Experience Survey, with the latest results showing an overall experience of 8.88 out of 10. In areas such as support for health and wellbeing, and information on financial help, results were positive but showed room for improvement. NHS England continues to support trusts to take action to improve cancer patient support.</p>

star this property answer text <p>The UK National Screening Committee (UK NSC) currently recommends that all women aged 50 to 71 years old are invited for a breast screening every three years. The AgeX trial is looking at the impact of increasing or decreasing the screening age for breast screening, and the UK NSC will review its findings when they are available.</p><p>The UK NSC can be alerted to any new published peer reviewed evidence which may suggest the case for a new screening programme, or changes to an existing one. This £3.2 million study, termed DecodeME, will analyse samples from 25,000 people with ME/CFS to search for genetic differences that may indicate underlying causes, or an increased risk of developing the condition. By helping us to understand ME/CFS better, this research has the potential to lead to new treatments for the condition.</p><p>Integrated care boards (ICBs) are responsible for commissioning specialist ME/CFS services that meet the needs of their population, subject to local prioritisation and funding. The process of commissioning services should take into account best practice guidance, such as the National Institute for Health and Care Excellence’s guidance on ME/CFS diagnosis and management, published in October 2021. In addition, in October 2023, the British Association of Clinicians in ME/CFS published the ME/CFS National Services Survey. This report provides insight into the services being delivered for adults, children, and young people with ME/CFS.</p>

star this property answer text <p>We are aware of supply issues with three pancreatic enzyme replacement therapies: Creon 10000 gastro-resistant capsules; Creon 25000 gastro-resistant capsules; and Nutrizym 22 gastro-resistant capsules. We understand that these are due to limited availability of active pharmaceutical ingredients, and manufacturing constraints in producing the volumes required to meet demand. The Department has issued guidance to healthcare professionals regarding treatment of patients while there is a disruption to the supply of these pancreatic enzyme replacement therapies. We are having regular conversations with the suppliers of these products, to ask that they expedite deliveries and increase production forecasts, and to confirm that they are taking action to address the root causes of the issues, to ensure continuity of supply. We are also working with specialist importers to source unlicensed imports from abroad.</p><p>Whilst we can’t always prevent supply issues, we have a range of well-established tools and processes to mitigate risks to patients. These include close and regular engagement with suppliers, use of alternative strengths or forms of a medicine to allow patients to remain on the same product, expediting regulatory procedures, sourcing unlicensed imports from abroad, adding products to the restricted exports and hoarding list, use of Serious Shortage Protocols, and issuing National Health Service communications to provide management advice and information on the issue to healthcare professionals, so they can advise and support their patients.</p>

star this property answer text <p>The Department proudly supports the Disability Confident Scheme, and has renewed its status as a Disability Confident employer. No single official works directly on the scheme, and in line with Civil Service guidance, we have moved away from standalone roles and teams to embed equality, diversity, and inclusion activities within existing human resource portfolios and roles. Activity to support the recruitment and retention of colleagues with a disability is undertaken across our recruitment, talent, and employee experience teams.</p><p>In the last calendar year, 11.3% of successful applicants applied under the Disability Confident Scheme. This is a slight increase from last year, at 10.9%. We additionally review our disability representation rates on a regular basis to monitor the impact of our recruitment practices and policy changes. We also have dedicated staff networks to support our disabled employees and celebrate achievements. Turnover of permanent employees who have declared they have a disability is currently approximately 6%, which is significantly lower than the average permanent staff turnover of colleagues who do not have a declared disability.</p><p>In 2023, we launched a new Workplace Adjustments policy, process, and passport. The new process is designed to ensure everyone in the Department can access the support they need as easily and quickly as possible via a person-centred approach that identifies and seeks to remove workplace barriers.</p><p>We also delivered several talent schemes that can assist in supporting the retention and development of disabled colleagues. This includes Beyond Boundaries, a 12-month programme for the Senior Executive Officer grade and below, to support candidates in reaching their full potential. As part of the programme, to support disability positive action, last year we ringfenced six job places out of 30, based on the proportionality of our disabled workforce, the People Survey, and recruitment data. We expect to take similar action this year.</p><p>In addition, the Future Leaders Scheme (FLS) is a 12-month accelerated development scheme for Grade 6 or 7 colleagues, who have the potential to progress to the Civil Service’s most senior and critical leadership roles. There is an option for those candidates who declare a disability or long-term health condition, or both, to enrol on the Disability Empowers Leadership Talent Association (DELTA) integrated scheme. In 2022, we had four candidates participate in DELTA out of the 21 successful FLS candidates, and two out of 26 in 2021, as per the data from the Cabinet Office. It should be noted that due to the sensitive nature of the programmes, not all participants are comfortable in being open about participation in DELTA, especially those with disabilities who have yet to disclose this to their colleagues or managers.</p><p>Finally, the Department’s mutual mentoring programme encourages underrepresented groups, including colleagues with disabilities, to mentor senior civil service colleagues to help increase the engagement and retention of underrepresented staff, and increase cultural intelligence in senior leaders.</p>

star this property answer text <p>Diagnostic services, including tests for coeliac disease, are provided in a range of settings, including community diagnostic centres (CDCs). We are expanding diagnostic capacity through the delivery of the CDC programme. NHS England assesses the delivery of diagnostic services, including by analysing regular management information for activity in CDCs as well as a national data collection for gastroscopy.</p><p>NHS England has also invested in senior clinical leadership in gastroenterology, which includes coeliac disease, as a priority workstream in the national Getting It Right First Time programme. However, it is the responsibility of integrated care boards (ICBs) to commission services to meet the needs of their local populace, including the diagnosis and ongoing management of coeliac disease.</p><p>The Government and NHS England are taking steps to support people waiting for appointments and surgery for long-term conditions, including those waiting for a coeliac disease diagnosis, by offering online support, increasing elective capacity, and encouraging personalised support.</p><p>NHS England launched the My Planned Care platform in February 2022, giving patients advice and support to manage their health, and avoid significant deterioration of symptoms whilst they wait for appointments. This online platform includes advice on physical and mental health. In addition, through the NHS Long Term Plan, the Government is providing record levels of investment and increasing the mental health workforce to expand and transform National Health Service mental health services in England, helping to reduce waiting times.</p><p>The National Institute for Health and Care Excellence promotes its guidance on coeliac disease, which includes information for the public via its website, newsletters, and other media.</p>

We are expanding diagnostic capacity through the delivery of the CDC programme. This online platform includes advice on physical and mental health. In addition, through the NHS Long Term Plan, the Government is providing record levels of investment and increasing the mental health workforce to expand and transform National Health Service mental health services in England, helping to reduce waiting times.</p><p>The National Institute for Health and Care Excellence promotes its guidance on coeliac disease, which includes information for the public via its website, newsletters, and other media.</p>

NHS England welcomed the recommendations of the recent independent review of the ambulance sector culture, Culture Review of Ambulance Trusts, which highlights the need to target bullying and harassment, including sexual harassment, and enable freedom to speak up. The charter currently has 329 signatories from across the health system, including ambulance trusts. Signatories commit to a zero-tolerance approach to any form of sexual misconduct in the workplace by implementing the charter’s ten actions and principles by July 2024.</p>

star this property answer text <p>Waiting list management information shows that there were 3,469 people waiting for more than 12 months for hip replacement surgery, and 5,714 people waiting for more than 12 months for knee replacement surgery, as of 5 May 2024.</p><p>The National Health Service has been hit by the most disruptive industrial action in its history, and as of April 2024, approximately 1.4 million hospital appointments have been rescheduled since December 2022. The NHS is working incredibly hard to ensure that those who have been waiting the longest receive treatment as soon as possible, and staff are going above and beyond to make this happen.</p> more like this

star this property answer text <p>The Government is committed to improving the lives of those living with rare diseases such as sickle cell disease, and published the UK Rare Diseases Framework in January 2021, providing the high-level approach for rare diseases. The framework outlines future priorities including increasing awareness of rare diseases among healthcare professionals. In England we publish action plans annually to address these priorities. As part of this plan we committed to developing an innovative digital educational resource, ‘GeNotes’, providing healthcare professionals with relevant and concise information to support patient management, linking to the NHS Genomic Test Directories, and signposting to extended learning opportunities. This resource includes information on sickle cell disease and is updated regularly.</p><p>There have been recent improvements to the haematology medical curriculum, with understanding sickle cell disease now described in the curriculum as a core competency. The National Healthcare Inequalities Improvement Programme has launched several products to support NHS England in raising awareness of sickle cell disease among healthcare professionals including NHS England’s Can you tell it's Sickle Cell campaign and e-learning module and a communications campaign to raise awareness of existing NHS England arrangements to support people with sickle cell disease to save money on the costs of regular prescriptions.</p>

star this property answer text <p>Improving early diagnosis of cancer, including prostate cancer, remains a priority for the Government and we are working towards the NHS Long Term Plan ambition of diagnosing 75% of stageable cancers at stage 1 and 2 by 2028. We know more people are surviving cancer in the United Kingdom than ever and that is partly down to early diagnosis.</p><p>Prior to the TRANSFORM trial, we are helping more people get diagnosed earlier by raising awareness, implementing the faster diagnosis standard and funding investment in research and diagnostics. NHS England takes steps to raise awareness of prostate cancer where there are opportunities to do so; for example NHS England partners with cancer alliances, charities and local representatives to reach people through projects in the heart of their communities. Cancer alliances have also been undertaking action to alert ‘at risk’ groups about prostate cancer, this is determined at a local level and so will vary depending on local needs and priorities.</p><p>To support faster diagnosis, NHS England is streamlining cancer pathways and in October 2022 introduced the best practice timed pathway for prostate cancer. This guidance recommends those with suspected prostate cancer undertake a multi-parametric magnetic resonance imaging (MRI) before biopsy, which ensures only those men most at risk of having cancer undergo an invasive biopsy.</p><p>In 2018, the Government announced £75 million of funding for clinical trials for prostate cancer, focusing on early diagnosis, survival rates and treatment options. Since April 2018, the National Institute for Health and Care Research (NIHR) has awarded over £60 million to more than 28 research studies relevant to prostate cancer. star this property answer text <p>Integrated care boards (ICB) commission services to meet the needs of their local populace. Diagnosis of coeliac disease falls within the portfolio of ICB-commissioned services. As such, NHS England does not have central accountability. However, NHS England has invested in senior clinical leadership in gastroenterology, which includes coeliac disease, as a priority workstream in the national Getting It Right First Time programme.</p><p>The diagnosis of coeliac disease commonly involves a blood test and, in some cases, a biopsy of the small intestine. Post-diagnosis, further blood tests, which check the levels of iron and other vitamins and minerals, may be required to assess how the condition has affected the individual, such as the potential for iron deficiency anaemia due to poor absorption of nutrients. A bone density scan may also be recommended if the condition is thought to affect the individual’s bones.</p> more like this

star this property answer text <p>NHS England has invested in senior clinical leadership in gastroenterology, which includes coeliac disease, as a priority workstream in the national Getting It Right First Time programme.</p><p>To support healthcare professionals in the diagnosis and management of coeliac disease, and improve the diagnostic pathway nationally, the National Institute for Health and Care Excellence (NICE) has produced the guidance, Coeliac disease: recognition, assessment and management [NG20].</p><p>NICE guidelines represent best practice, and both healthcare professionals, including general practitioners, and service commissioners are expected to take them fully into account. Guidelines published by the NICE are not mandatory, and do not replace the judgement of clinicians in determining the most appropriate treatment for individual patients.</p><p>To assist with increasing the visibility of coeliac disease, the NICE promotes guidance via its website, newsletters, and other media. It also publishes information for the public, which explains the care people with coeliac disease should receive, as set out in the NICE guideline. Information for the public on coeliac disease is also published by the National Health Service and is available on the NHS website.</p>
