<p>Information on the implementation of the High Impact Change Model (HICM) is collected
through Better Care Fund quarterly reporting returns, in which Health and Wellbeing
Boards (HWB) self-assess their progress towards implementing the HICM. The most recent
information available is from Quarter 2, 2018-19 (July-September 2018) which shows
the number of HWBs areas reporting at the ‘Established’ level (or better) for each
of the elements of the HICM. The information for Q4, 2017-18 covering the month of
