Patients may choose between National Health Service or independent providers. The My Planned Care platform was launched by the NHS in February 2022, which provides access to information on local average waiting times at a speciality level, excluding cancer, for all hospital trusts in England. NHS England intends to develop My Planned Care to provide a hub for patients for the entire treatment pathway and develop further integration with the NHS App.</p> more like this

star this property answer text <p>Patients can apply to register with the general practitioner (GP) practice of their choice. All practices are required to assign to registered patients a named, accountable GP to ensure that any necessary services the practice is contracted to provide are coordinated and delivered for patients. Practices must make reasonable efforts to accommodate patients’ requests to be assigned a specific accountable GP and endeavour to comply with all reasonable requests to see a particular practitioner. However, patients may have the choice between a named GP or increased speed of access. The right to choose a GP practice and to express a preference for a particular doctor within that practice are reflected in the NHS Constitution.</p><p>We have also committed to publish practice-level appointment data, which will improve transparency on the performance of a GP practice and allow patients to make informed choices when registering.</p> more like this

This will be a minimum of five providers where possible. Patients will also be informed of their right to choose and encouraged to raise this at the time of the referral.</p><p>Since the commitments made by the Government in May 2023, we have seen an increase in the number of patients being offered choice at the point of referral. The Department does not hold data regarding patients choosing between a hospital and clinic for their first outpatient appointment each month. The Department has also not made an estimate of how many patients were aware, before visiting their GP, of their right to choose the provider for their first outpatient appointment.</p><p>In autumn last year, the Department ran a press partnership campaign to raise awareness and increase understanding of choice. Furthermore, the Department has recently launched a marketing campaign, aimed at all adults, to increase awareness of a patient's right to choose where they receive their hospital care, as well as encourage more people to actively make a choice.</p>

star this property answer text <p>NHS England is working to secure a sustained improvement in awareness, offer, operation and take up of choice by 2020. This includes supporting the independent Maternity Services Review, which is due for publication early in the New Year, and taking forward any recommendations that relate to choice; improving the quality of End of Life Care and the choices people should have about where they receive their care and where they choose to die; strengthening choice in both physical and mental health elective services, to ensure that the NHS honours patients’ legal rights to choice of who provides their care; and securing choice in the design and implementation of new care models for the future NHS.</p><br /><p>NHS England has brought this work together with their programmes on personal health budgets and integrated personal commissioning, under a new Director and with additional resource, to accelerate the work in all of these areas in support of the Five Year Forward View commitment to deliver greater patient empowerment. Improvements will be assessed through a new national assessment framework for clinical commissioning groups.</p>

star this property answer text <p>Quarterly Choose and Book utilisation data is available for each month and each quarter of the 2013-14 financial year and is set out in the attached table.</p><p> </p><p>When reviewing the table it is important to note that utilisation is calculated as the percentage of referrals made to first consultant-led outpatient services using the Choose and Book system, compared to the total number of referrals made to first outpatient services, as reported by provider organisations.</p><p> </p><p>Each month this represents around 500,000 referrals being made from general practitioner (GP) practices to first consultant-led outpatient services through Choose and Book. Use of Choose and Book is, however, significantly greater than this with an additional 200,000 or so referrals per month being booked to other outpatient services, which include Allied Health Professionals, GPs with Special Interests diagnostic and assessment services. These are not currently included as part of the utilisation figures as the denominator data is not currently reported at a national level.</p><p> </p>

star this property answer text <p>The NHS e-Referral Service (e-RS) replaced the choose and book system in June 2015. The NHS e-RS is designed to allow patients and healthcare professionals to select appointments with providers either online or by phone.</p><p> </p><p>An attempt to make a booking may be unsuccessful if there is no appointment for the required clinic or hospital service available on e-RS. If no appointment is available for a patient or general practitioner (GP) to book on e-RS, the referral is forwarded (via the Manage Your Referral application) or deferred (via the professional application) to the patient’s chosen provider to enable the provider to book the patient an appointment.</p><p> </p><p>The relevant provider is contacted informing them that a patient requires an appointment and the provider will then contact the patient in relation to an appointment. star this property answer text <p>The decision to place a patient on a Patient Initiated Follow Up (PIFU) pathway will be taken jointly by the patient and a clinician when the clinician identifies a patient who would benefit from accessing care in this way. The decision will always be based on a patient’s individual needs and circumstances, ensuring it is in the patient’s best interests.</p><p>The Outpatient Transformation Programme provides ongoing advice and support to systems and providers implementing PIFU. This includes continuous improvement and identifying any risks, such as those relating to health inequalities and exclusion and working to provide resolution/guidance on how to address them. As part of this national support offer, resources have been made available that encourage systems and providers to develop and regularly review their Equality Health Impact Assessments in relation to PIFU.</p> more like this

In 2021/22, 50% of 7,000 patients surveyed reported that they were aware of the choice of hospitals or clinics for an appointment. The Survey’s results are available to integrated care boards and NHS England to review in order to meet its responsibilities relating to patient choice.</p><p> </p><p>Use of the e-RS for referrals from general practitioners for consultant-led first outpatient appointments is mandated in the latest NHS and GP Standard Contracts.</p> more like this

These have been utilised previously at the point of referral to allow referring clinicians to view capacity within providers. The re-introduction of this option requires further work to ensure there are no unintended consequences and regional oversight with local system commissioners to ensure provision for patient referrals is always available.</p> more like this

star this property answer text <p>The National Health Service Commissioning Board and Clinical Commissioning Groups Responsibilities and Standing Rules Regulations 2012 state that integrated care boards and NHS England, in exercising its commissioning functions, have a duty to ensure that the awareness and availability of choice is publicised to referrers and patients. NHS England has regulatory oversight and operational implementation of patient choice and provides advice and guidance to the public, patients, referrers, commissioners and providers on the choices available in England and how patient choice can be facilitated.</p> more like this

star this property answer text <p>The Government wants to make it easier and quicker for everyone to get the help they need from primary care and reduce digital inequalities when they access care. The Delivery Plan for Recovering Access to Primary Care, published on 9 May 2023, describes the implementation of a new Modern general practice access model, which will make it easier for everyone to contact their practice and it will make sure arrangements for care happen on the same day. This will ensure we move away from a ‘first come, first served’ approach towards a more equitable one that benefits all patients, regardless of their chosen route of access.</p><p>To combat digital exclusion, the public need to know that the new access model combines the flexible use of telephone, digital and in person access choices for patients. That is why the delivery plan includes a major national communications campaign to explain the evolving nature of primary care how best the public can use the National Health Service. The plan commits £240 million of re-targeted funding for better digital tools and training, which will improve digital access routes, freeing up capacity for those patients who still want or need to contact their practice by telephone or in person.</p>

star this property answer text <p>The 2014-15 Choice Framework and the NHS Constitution, both available on .gov.uk, set out when patients have legal rights to choice.</p><p> </p><p>The NHS Constitution states that patients have the rightto make choices about the services commissioned by National Health Service bodies and to information to support these choices.</p><p> </p><p>The 2014-15 NHS Choice Framework establishes that if a patient needs to see a consultant or specialist as an outpatient for a physical or mental health condition, they can choose the organisation that provides their NHS care and treatment anywhere in England for their first outpatient appointment. They can also choose which consultant-led team or which mental health team led by a named health care professional will be in charge of their NHS care and treatment for their first outpatient appointment.</p><p> </p><p>The organisation can be any clinically appropriate health service provider with whom any clinical commissioning group or NHS England has a commissioning contract for the service required as a result of the referral, but the team must be clinically appropriate and led by a named consultant or health professional who is employed or engaged by that health service provider.</p><p> </p><p>There are also times that patients are not able to make a choice, and these are outlined in the <em>Choice Framework</em> and the <em>Handbook to the NHS Constitution</em>. For example, patients can only choose a hospital or clinic that offers the right treatment and care for their condition. Furthermore, if patients need urgent or emergency treatment, they cannot choose who they see.</p><p> </p><p>If a patient, who is not covered by the exemptions, has not been offered choice, or denied the opportunity to exercise choice by a clinical commissioning group, the 2014-15 Choice Framework sets out a clear complaints procedure.</p>
