unstar this property answer text <p>Health visitors are part of the nursing profession and regulated by the Nursing and Midwifery Council (NMC). Health visitors are qualified and registered nurses or midwives who have undertaken an approved training programme in Specialist Community Public Health Nursing - Health Visiting.</p><p>The NMC has the general function of promoting high standards of education and coordinating all stages of education to ensure that health visitors are equipped with the knowledge, skills and attitudes essential for professional practice, including continuous professional development (CPD). The NMC set the standards of education and training for the nursing profession expected at undergraduate and postgraduate level and approve curricula set by individual higher education institutes. In so doing, they set out the knowledge, skills and behaviours that both postgraduate and undergraduate nurses must be able to demonstrate.</p><p>CPD is a term used to describe a number of different training and development activities which may be required for the purposes of professional practice in the National Health Service. The NMC requires 35 hours of CPD relevant to scope of practice in the three year period since registration was last renewed, or joining the register.</p><p>The NMC are in the process of rewriting their standards. For example, in the new pre-registration nursing standards the NMC specify that registrants must have proficiencies in identifying risk and vulnerability, in all settings and across the lifespan and would expect post-natal depression to be included in the curriculum.</p>

Local authorities are responsible for commissioning appropriate public health services for 0-5 year olds in their areas, including health visiting.</p><p> </p><p>Provision by local authorities of five universal health visitor reviews for children aged 0-5 was required by Regulations which came into force on 1 October 2015. The Department commissioned Public Health England (PHE) to undertake a review of the Regulations at 12 months.</p><p> </p><p>PHE has completed the review of the mandating of universal health visiting services. The review was informed by looking at existing national and local statistics on health visiting service delivery and health and wellbeing outcomes for young children. In addition, feedback from a number of different stakeholder perspectives, including health visitors, was sought.</p><p> </p><p>PHE’s report is informing next steps decisions about the service and the Department expects to announce these shortly and to publish the report.</p>

unstar this property answer text <p>The Health Visitor Minimum Data set collection, that was established in 2012 to collect statistics on the number of health visitors employed by National Health Service provider organisations as well as those employed by local authorities, or other care providers was approved for the duration of the national programme only and ceased at the end of September 2015.</p><p> </p><p>The vast majority of health visitors are employed in NHS organisations. Health visitors are also employed by other providers including independent sector and local authority in house provision. From statistics published by NHS Digital, as at 30 September 2015, the number of full-time equivalent (FTE) health visitors directly employed in NHS organisations was 10,236 and the number of health visitors (FTE) employed by local authorities and/or other care providers was 518.</p><p> </p><p>In September 2015, NHS Digital began to collect and publish data on staff, including health visitors, in some independent sector healthcare organisations in England. This collection does not represent the entire workforce employed across the whole of the independent sector, nor does this data only show staff providing NHS commissioned services.</p><p> </p><p>The latest available data, published in September 2016, showed, as at 31 March 2016 that there were 924 FTE health visitors in those independent sector organisations which provided data.</p>

Health visitors, with other professionals, have an essential role supporting babies and their families.</p><p>The Interim People Plan, published in June, sets out the vision for people who work in the NHS to enable them to deliver the NHS Long Term Plan. Within this are specific plans for supporting community nurses, through growing pre-registration clinical placement capacity, a clear emphasis on high-quality learning experiences being provided within community settings. There is also a clear commitment to work with partners to identify how to support growth in the community workforce including district nursing, general practice nursing, health visitors and school nursing.</p><p>A Specialist Community and Public Health Nurse apprenticeship (Level 7) is currently in development. This will offer an alternative route directly into the health visiting profession. There are also nursing apprenticeship pathways currently in place to become a Registered Nurse, following which an individual has the option to choose to complete a postgraduate course to go onto qualify as a health visitor.</p><p> </p><p> </p>

unstar this property answer text <p>From 1 October 2015, the Government intends that local authorities take over responsibility from NHS England for commissioning of public health services for children aged 0-5. This includes health visiting services and Family Nurse Partnership targeted services for teenage mothers. Local authorities will receive funding for these new responsibilities.</p><p> </p><p> </p><p> </p><p>The Department is aiming to set out 0-5 funding allocations later in the year. Work is on-going to determine how much money will transfer and commissioning costs will be considered as part of the overall approach. Funding in future years will be calculated in the light of the Government’s next spending review.</p><p> </p><p> </p><p> </p><p>Locally NHS England’s area teams and local authorities are working to develop plans, including timescales, for a smooth and effective transfer of the commissioning role. A programme of regional 0 to 5 years commissioning transfer events is underway, involving key stakeholders to enhance understanding of future working arrangements.</p><p> </p>

The ‘Sure Start children’s centres statutory guidance’*, provides guidance on what local authorities and commissioners of local health services should do when fulfilling their statutory responsibilities and includes guidance on the role of health visitors. In particular it states that, “Local authorities should consider how they can use their network of children’s centres to greatest effect through links with other services, including: through links to midwifery, general practitioners and, health visitors. Local authorities are responsible for commissioning appropriate services for 0-5 year olds in their areas, including health visiting. Public Health England monitors a range of outcomes for child health. This includes information provided by local authorities for health visiting services high impact actions such as reducing smoking in pregnancy and managing a healthy weight in 4-5 year olds. The majority of these show steady state of improvement[1].</p><p> </p><p>The Health Visitor Programme, which concluded in 2015, introduced a new service model for the profession to help develop improved access, user experience and outcomes, as well as reducing health inequalities. The Health Visitor programme also saw the workforce increase considerably enabling more families to be reached. The High Impact Areas focus on key areas where health visitors have a significant impact on health and wellbeing and improving outcomes for children, families and communities.</p><p> </p><p>The six High Impact Areas are:</p><p>1. Transition to parenthood and the early weeks</p><p>2. Maternal mental health</p><p>3. Breast feeding</p><p>4. Healthy weight including physical activity</p><p>5. unstar this property answer text <p>The Department, NHS England, Public Health England and the Local Government Association are working closely together to develop a clear process to ensure the smooth and safe transfer of commissioning 0 to 5 years public health services, (including health visitor service), from the National Health Service to local authorities. In particular, the Department has established a Task and Finish Group to develop a comprehensive transfer plan comprising the key workstreams involved in the transfer.</p><p> </p><p>Health Education England (HEE), in its December 2013 publication, <em>Investing in People for Health and Healthcare: Workforce Plan for England Proposed Education and Training Commissions for 2014/15</em> , has set out its approach to training health visitors in the longer term, stating: “The job of HEE this year and in future years is to maintain the Health Visitor workforce at these new historically high levels.”</p><p> </p><p>At the national level, the Children's Health and Wellbeing Partnership will provide national leadership on specific challenges, (such as the transition of the 0–5 service), by bringing together key national partners, so they can jointly contribute to ongoing improvement of outcomes for children and young people.</p><p> </p>

As a result, it is not possible to provide accurate data on the total size of the health visitor workforce.</p><p> </p><p>NHS Digital’s NHS Hospital and Community Health Service workforce statistics capture the total number of health visitors directly employed in NHS trusts and CCGs in England and by Health Education England region.</p><p>NHS Digital used to collect data on health visitors employed by local authorities and other organisations through the Health Visitors National Minimum Dataset however this collection ceased at the end of September 2015.</p> more like this

unstar this property answer text <p>From 1 October 2015, local authorities (LAs) took over responsibility for commissioning public health services for children aged 0-5 from NHS England. Whilst Public Health England (PHE) collects data on coverage, LAs are responsible for local delivery which includes coverage and activity.</p><p> </p><p>In 2010 the Government committed to growing the health visitor workforce and transforming health visiting services to improve health outcomes and reduce inequalities. Since 2010, around 7,500 health visitor training places have been commissioned representing a fivefold increase in the number of health visitor trainees starting training and allowing greater access to services and support for families.</p><p> </p><p>The Department and PHE have produced guidance to support both commissioners and local providers including Guidance to support the commissioning of the Healthy Child Programme 0-19: Health Visiting and School Nursing services. The materials are based on evidence and good practice. PHE and the Department continue to work with the Local Government Association, professional bodies, and locally through PHE Centres to share evidence and guidance that allows LAs to make the best decisions to meet local needs.</p>

unstar this property answer text <p>The additional 4,200 health visitors will all be providing front line services to families. Health visitor services are currently commissioned by NHS England as part of Section 7A of the NHS Act 2006*. NHS England’s Local Area Teams determine the most suitable local service provider. Currently, most provision is through a variety of National Health Service organisations, with a small proportion delivered by other organisations, including social enterprises. The trajectory for delivering growth of the health visitor workforce is not related to the type of provider. All services are based on a national model of health visiting and the NHS England service specification.</p><p> </p><p>NHS England Area Teams and local authorities are working together in co-commissioning these services, prior to the lead commissioning role moving to local authorities in October 2015.</p><p> </p><p>All health visitors, including the additional 4,200 being delivered by 2015, receive education and training on maternal mental health, so they can detect the early signs of postnatal depression and ensure all women get support during and after birth.</p><p> </p><p>The NHS England 2014-15 service specification emphasises throughout, the role of the health visitor in promoting maternal mental health within clearly defined pathways. It makes clear that health visitors are expected to be trained and developed to be competent in delivering the evidence-based assessments and interventions set out in the Healthy Child Programme. In addition, NHS England is resourcing Area Teams to support delivery of the new model of health visiting which explicitly includes maternal mental health as a high impact area.</p><p> </p><p> </p><p> </p><p> </p><p> </p><p>To supplement core training, health visitors are able to enhance their skills and knowledge in the context of their specific roles. The Department has earlier this year, commissioned the training of 375 perinatal mental health champions across England. The champions are cascading their training in local areas so as to facilitate the spread of knowledge and practice among colleagues. In addition to the champions’ training, all health visitors are able to access perinatal mental health e-learning modules.</p><p> </p><p><em>Note:</em></p><p> </p><p>*As amended by the Health and Social Care Act 2012.</p><p> </p>

unstar this property answer text <p>Since 1 October 2015 local authorities have had responsibility for the commissioning of public health services for children aged zero to five (including health visitor services).</p><p> </p><p>The Department commissioned Public Health England to undertake two reviews focused on the transfer of health visiting services to local authority control: a review of the mandated elements of service, and a programme benefits realisation review.</p><p> </p><p>The Review of mandated services demonstrated widespread support to continue mandating the provision of health visitor reviews to families, to ensure that all families are given support at critical times and those families needing more help are identified and early interventions put in place. The report on the review of mandation for the universal health visiting service can be viewed at:</p><p> </p><p><a href="https://www.gov.uk/government/publications/universal-health-visiting-service-mandation-review" target="_blank">https://www.gov.uk/government/publications/universal-health-visiting-service-mandation-review</a></p><p> </p><p>The Benefits Realisation Review demonstrated statistically significant improvements in access, parent reported experience, a number of child health outcomes and a contribution to addressing inequalities. unstar this property answer text <p>From 1 October 2015, the Government intends that local authorities take over responsibility from NHS England, for commissioning public health services for children aged 0-5, including health visiting services.</p><p>The Department is working collaboratively with its partners locally and nationally on the transfer of commissioning responsibilities. At local level, NHS England’s area teams, as the local commissioners (not clinical commissioning groups), and local authorities are working to develop plans, including timescales, for a smooth and effective transfer of the commissioning role.</p><p>The Government will mandate the delivery of the key child assessment elements of the Health Child Programme which are led by health visitors, healthcare professionals and their teams. This will allow for the service to be shaped locally, whilst ensuring there is universal coverage of the assessments in the context of a national, standard format (or specification) for the services, and hence that families can be confident they will receive health visiting support.</p><p>The Department and its partners use a range of measures that support this transition and convey associated timescales and guidance. In particular, the service specification, “Public Health Functions to be exercised by NHS England”, published in November 2013, sets out expectations for the commissioning and provision of services and how area teams can work with local authorities ahead of the transfer of commissioning responsibilities in 2015.</p><p>Other mechanisms such as the forthcoming programme of regional 0 to 5 years commissioning transfer events, organised in conjunction with the Local Government Association, provide the means for local staff and key stakeholders to further develop their understanding of working together in respect of future arrangements for the provision of health visitor services.</p><p> </p>

unstar this property answer text <p>Following a review led by the Department, my Rt. hon. Friend the Secretary of State for Health and Social Care has confirmed that local authorities will continue to commission sexual health, school nurses and health visitors. The review found that local authorities take an active and efficient approach to commissioning services and recognised the steps that local authorities have taken to improve and modernise the services they commission. The review recommended that the National Health Service work much more closely with local authorities on public health so that commissioning is more joined-up and prevention is embedded into a wider range of health services. The Department will be seeking views in the forthcoming Prevention Green Paper about how action can be taken forward.</p><p>My Rt. hon. unstar this property answer text <p>Since October 2015, local authorities have been responsible for the commissioning of services for zero to five-year olds and as such, they determine the required numbers of health visitors based upon local needs.</p><p> </p><p>A Specialist Community and Public Health Nurse apprenticeship (Level 7) is currently in development. This will offer an alternative route directly into the health visiting profession. There are also nursing apprenticeship pathways currently in place to become a Registered Nurse, following which an individual has the option to choose to complete a postgraduate course to go onto qualify as a health visitor.</p><p> </p><p>The interim People Plan, published in June 2019, sets out the vision for people who work in the National Health Service to enable them to deliver the long-term plan. There is also clear commitment to work with partners to identify how to support growth in the community workforce including district nursing, general practice nursing, health visitors and school nursing.</p><p> </p>

The number of health visitors employed by National Health Service organisations reported through Electronic Staff Records shows reduction, but this is not a complete picture of the workforce as this goes not include social enterprises, private sector organisations or local authorities. It is the responsibility of local authorities to determine how the programme is provided to their communities based upon local needs.</p><p>Data on mandated health visitor reviews shows national coverage remains above those achieved in 2015. However, there is variation at a local level. The latest figures relate to October to December 2019, the statistics relate to all children resident in a local area, characteristics of the child such as if they are on child in need or child protection plans are not recorded. Annual data for 2019/20 will be published in February 2021.
