	answer	answer is ministerial correction date of answer 20/03/2018 answer text <p>The fit and proper person requirement requires all registered health and adult social care providers to make sure board directors, board members and individuals who perform the functions equivalent to the functions of a board director and member, are fit for their roles. The Care Quality Commission (CQC) assesses whether registered providers have appropriate processes in place for this. In this regard the CQC does not look at individuals, an approach which would extend to independent reviewers.</p><p> </p><p>The statutory duty of candour applies to organisations, rather than to individual members of staff. All regulated healthcare professionals working in the United Kingdom have an individual professional duty of candour, which is a responsibility to be open and honest. This responsibility is set out in their respective professional codes of conduct. 	answer	answer is ministerial correction date of answer 31/01/2018 answer text <p>The National Institute for Health and Care Excellence guidance Chronic kidney disease in adults: assessment and management, updated in July 2014, sets out best practice for clinicians in the diagnosis and management of chronic kidney disease (CKD). The guidance covers identification and monitoring of patients at risk; pharmacological management and referral where appropriate, and aims to ensure patients remain healthy and avoid kidney failure, if possible. The guidance can be found at the following link:</p><p> </p><p><a href="http://www.nice.org.uk/guidance/cg182" target="_blank">www.nice.org.uk/guidance/cg182</a></p><p> </p><p>The NHS Health Check is a national programme (launched in 2009) for everyone between the ages of 40-74 years (not already on a chronic disease register) that assesses people’s health and risk of developing certain health problems. At launch it was estimated that the programme could detect each year at least 20,000 cases of diabetes or kidney disease earlier, allowing individuals to be better managed and to improve their quality of life. The first major evaluation of the NHS Health Check, published in 2016 and covering 665 general practitioner practices, demonstrated positive results, estimating that over the first five years it has identified a new case of chronic kidney disease in every 265 appointments. 	answer	answer is ministerial correction date of answer 01/05/2018 answer text <p>The Healthcare Safety Investigation Branch (HSIB) began work in April 2017 to conduct high-level investigations of serious patient safety incidents in the National Health Service in England with a specific focus on system-wide learning and improvement.</p><p> </p><p>In November 2017, my Rt. hon. Friend the Secretary of State announced that, from April 2018, HSIB would investigate all cases of early neonatal deaths, term intrapartum stillbirths and cases of severe brain injury in babies, as well as all cases of maternal death in England. HSIB did not conduct investigations in these areas in 2017.</p><p> </p><p>The new investigative approach will begin in a single region from April 2018 and rollout to all areas of England will be completed by the end of March 2019. It is estimated that there are approximately 1,000 cases of birth-related deaths or serious brain injuries in babies in England every year. The expectation is that the learning from investigations will spur system improvements leading to fewer deaths and injuries in the future.</p><p> </p><p>The Secretary of State has been clear that the HSIB maternity investigations will involve patients and families in investigations. In carrying out the maternity investigations, the HSIB will consult and seek evidence or information from the patient, family members and staff involved in the care. They are also responsible for ensuring their employees receive appropriate support, training and professional development, to enable them to carry out the duties they are employed to perform.</p><p> </p><p>The role of the Care Quality Commission (CQC) is not to assess work place training within the care sector or if the correct training provision is provided.
