star this property answer text <p>NHS Digital publishes annually the average daily rate of delayed transfers of care from National Health Service hospitals in England per 100,000 local authority population aged 18 years and over. Rates are shown separately for all delays, and for delays attributable to social care, the National Health Service, and jointly to social care and the NHS.</p><p>The most recently published data are for 1 April 2015 to 31 March 2016 and are attached due to the size of the data.</p><p>Information is not collected centrally on the number of successful discharges by council area. In 2015-16, there were 16.5 million finished discharged episodes of NHS commissioned care from hospitals in England.</p> more like this

star this property answer text <p>From April 2010, NHS England started to publish quarterly information on the numbers of available and occupied beds open overnight and day only in National Health Service organisations. Previously, annual data was published. A table showing the numbers of NHS hospital beds is attached owing to the size of the data.</p><p> </p><p>There has been a long-term reduction in the number of general and acute beds open overnight because hospitals are dealing with patients more efficiently. The NHS now performs more day cases, length of stay is decreasing and more people are treated in community settings.</p> more like this

star this property answer text <p>NHS England compiles and publishes monthly delayed transfers of care data. The data is available by each local authority and National Health Service trust, and includes the reason for the delay against a defined set of 10 categories. The data set is published in an online-only, interactive format.</p><p> </p><p>Delays have multiple causes and as a result there is variation between local authorities. A wide-ranging package of support is in place to help areas improve transfers out of hospital and reduce delays, including work as part of Accident and Emergency Improvement Plans to understand and address delays in hospital discharge and promote good practice.</p><p> </p><p>The NHS and local government have also developed new resources, including a high impact change model for reducing delayed transfers of care. In addition, the Emergency Care Improvement Programme provides tailored support to local systems to support rapid and sustained improvement in performance.</p> more like this

star this property answer text <p>The Department has made no formal estimate of the costs of delayed discharge to the National Health Service. However, the Department and NHS Improvement are working together to implement Lord Carter’s recommendations on hospital productivity. The report, <em>Operational productivity and performance in English NHS acute hospitals: Unwarranted variations </em>is attached. As part of this, the two organisations are working with providers to develop a richer dataset around all aspects of the patient pathway, including discharge.</p><p> </p><p>At the same time, the Department continues to work closely with the NHS and local government to help local areas improve transfers out of hospital, share best practice, and reduce unnecessary delays.</p><p><strong> </strong></p><p>The Care Act 2014, placed duties on local authorities to facilitate and encourage an effective market of quality providers in their area. The Department has worked with local government and the provider sector to develop guidance to help local authorities with these market shaping duties, a copy of guidance, <em>Adult social care market shaping </em>is attached. The Department also supports the Local Government Association with a programme of sector-led improvement.</p>

star this property answer text <p>To date, the Department has made no formal estimate of the costs of delayed discharge to the National Health Service. As part of this, the two organisations are working with providers to develop a richer dataset around all aspects of the patient pathway, including estimates of the cost of delayed discharge.</p><p> </p><p>At the same time, the Department continues to work closely with the NHS and local government to help local areas improve transfers out of hospital, share best practice, and reduce unnecessary delays. We are funding the NHS’s own plan for the future with £10 billion and we are giving local authorities access to up to £3.5 billion of new support for adult social care by 2019/20.</p><p> </p><p>Since April 2015, the Government’s £5.3 billion Better Care Fund has provided much needed investment in better integrated care through locally developed plans and by putting resources where the local NHS and social services think they are needed. Alongside this we are working with local areas to improve the transfer of patients back into the communities.</p><p> </p><p>In 2016-17, there is a new Better Care Fund requirement on local areas to develop a clear, focused action plan for managing delays, including locally agreed targets. The requirement is designed to reduce delays across the health and care system.</p>
