The report has identified specific steps the British Government can take to address the issues faced by persecuted Christians around the world. I am pleased to confirm that the government has accepted the recommendations in full and work will now begin to implement them. This work will include exploring how best to deliver a new Security Council Resolution on protecting Christians in North Africa and the Middle East; and sanctioning those who persecute people for holding a religion or belief.</p> more like this

The Department cannot comment on individual employment matters that are the responsibility of independent organisations, and that are subject to ongoing legal proceedings or on matters relating to individual clinical cases or other personal information. We understand that the cost to East Lancashire Trust of legal proceedings concerning Mr Agrawal is £296,848.42 to date, and that the Trust does not have insurance costs for legal awards in the event of the Trust or one of its employees being subject to legal action for defamation.</p><p> </p> more like this

FAEs are counted against the year or month in which the admission episode finishes. Admissions do not represent the number of patients, as a person may have more than one admission within the period.</p><ol start="2"><li>Primary diagnosis</li></ol><p>The primary diagnosis is the first of up to 20 (14 from 2002-03 to 2006-07 and 7 prior to 2002-03) diagnosis fields in the HES data set and provides the main reason why the patient was admitted to hospital.</p><ol start="3"><li>ICD-10 codes used</li></ol><p>F00-F99: Mental and behavioural disorders</p><ol start="4"><li>Provisional data</li></ol><p>The data are provisional and may be incomplete or contain errors for which no adjustments have yet been made. Counts produced from provisional data are likely to be lower than those generated for the same period in the final data set. This shortfall will be most pronounced in the final month of the latest period, ie November from the (month 9) April to November extract. It is also probable that clinical data are not complete, which may in particular affect the last two months of any given period. There may also be errors due to coding inconsistencies that have not yet been investigated and corrected.</p>

star this property answer text <p>I have no role in decisions about individual chief executives or other staff in National Health Service trusts and foundation trusts (FTs). Oversight of trusts is primarily the responsibility of NHS Improvement, which provides updates to Ministers and the department as appropriate. Since the Mazars report and the report issued by the Care Quality Commission (CQC) in April 2016, NHS Improvement has been liaising closely with Southern Health, supporting the trust to address the issues set out in both reports.</p><p> </p><p>In response to the Mazars report into Southern Health, I have asked NHS England, NHS Improvement and the CQC to take a number of steps to address the issues raised. As a result, the CQC and NHS Improvement are working to support NHS trusts and FTs to better understand their avoidable mortality and to then take effective action to improve safety; the Department has published independently assured, Ofsted-style ratings of the quality of care offered to people with learning disabilities for all 209 clinical commissioning group areas; and, NHS England commissioned the first Learning Disability Mortality Review Programme to support local areas to review deaths of people with learning disabilities and act on the information to improve care.</p>

Learning from previous years, we have put this money into the NHS baseline for 2015-16 so that the NHS can plan effectively at local level for the long-term and take earlier action to tackle the symptoms of seasonal pressures.</p><p>There will be no further additional money for the NHS ahead of winter.</p> more like this
