answer text <p>Clinical commissioning groups (CCGs) should have regard to the National Institute for Health and Care Excellence (NICE) guideline on the assessment and treatment of people with fertility problems when commissioning services for which they are responsible. This includes the recommendation in the guideline that three cycles of in-vitro fertilisation be offered to qualifying couples where the woman is under the age of 40 and one cycle where the woman is between the ages of 40 and 42.</p><p> </p><p>The NICE guideline is evidence based best practice for clinicians but is not mandatory. The availability of National Health Service funded fertility treatment is matter for local determination. Decisions on the level of service provision are underpinned by clinical insight and knowledge of local healthcare needs and priorities.</p><p> </p><p>Information about the commissioning of fertility services is not collected centrally but an annual survey is undertaken by Fertility Fairness. answer text <p>Public Health England has run a number of “Be Clear on Cancer” (BCOC) campaigns to help improve earlier diagnosis. In 2014, BCOC conducted a regional ovarian cancer campaign and in 2017, a pilot which focussed on a range of abdominal symptoms, such as diarrhoea, bloating and discomfort that can be indicative of a number of cancers, including ovarian cancer. Decisions on which cancers the BCOC campaigns should focus on are informed by a steering group and the outcomes of the regional and pilot campaigns will be considered for 2019/20.</p><p> </p><p>Nationally, NHS England is testing innovative ways of diagnosing cancer earlier, with sites piloting multi-disciplinary diagnostic centres for patients with vague or non-specific symptoms. The pilots are taking place in London, Greater Manchester, Leeds, Bristol, Oxfordshire and Airedale, Wharfedale and Craven (Yorkshire). The Prime Minister’s October 2018 announcement pledged to roll out these rapid diagnostic centres across the country to offer patients a range of tests in the same day with rapid access to results. The centres will help make a huge improvement in early diagnosis.</p>

answer text <p>Following public consultation, NHS England published new service specification<sup>1 </sup>for specialised gender dysphoria services in September 2018. The new service specifications will be used in a national process of procurement later in the year that will determine which organisations are best able to deliver gender dysphoria services from 2019/20. answer text <p>The United Kingdom supplier of EpiPen 0.3mg adrenaline auto-injectors, Mylan, has estimated that they are currently able to supply approximately 50% of their normal demand for this product. Within the UK there are two alternative 0.3mg adrenaline auto-injector devices and the manufacturers of these have been able to increase their supplies and support the market during this time. We therefore expect that patients will continue to be able to access 0.3mg adrenaline auto-injector devices if prescribed. The Department is working very closely with all the manufacturers of adrenaline auto-injectors, the Medicines and Healthcare products Regulatory Agency, NHS England, and others to try to resolve these issues and improve the situation as quickly as possible.</p> more like this

answer text <p>Paediatric autoimmune neuropsychiatric disease (PANDAS) is currently a research and clinical area of interest for professionals working with children and young people. There is evidence that exposure to an infection can act as a trigger for some children to present acutely with a range of neurodevelopmental and mental health symptoms such as tics, obsessive compulsive disorder, attention deficit hyperactivity disorder and eating problems etc.</p><p> </p><p>However, there is inconclusive research evidence to demonstrate that PANDAS is a discrete disease entity. It is therefore not listed as a diagnosis by the International Statistical Classification of Diseases and Related Health Problems or the Diagnostic and Statistical Manual of Mental Disorders.</p> more like this

answer text <p>Everolimus in the treatment of refractory epilepsy associated with tuberous sclerosis complex has been through the NHS England clinical policy development process and was considered by the Clinical Priorities Advisory Group (CPAG) as part of the May 2018 prioritisation round. CPAG determined that the treatment fell into the lower end of five levels of relative priority for funding, and therefore the decision has been taken to not routinely commission the treatment at this time.</p><p> </p><p>Everolimus will be considered again at the next prioritisation round in November and, should it be successful, funding would be made available from April 2019. One factor that can significantly influence the relative priority of a treatment is the price put forward by the manufacturer. In advance of Everolimus being considered again in November, the manufacturer has been given the opportunity to offer a revised price and submit any further evidence that may have become available since the last prioritisation round.</p>
