answer text <p>There is general consensus that a number of factors are likely to drive increases in healthcare activity and associated spending. These factors include but are not limited to long-term demographic changes. For example, in its Fiscal Sustainability Analytical Paper on Public Spending on Health (2016), the Office for Budget Responsibility (OBR) found that health spending is driven by a multitude of factors, broadly grouped under demographic factors, changes in income and other cost pressures. Using a range of evidence to inform their analysis, the report found that historically, spending in healthcare has been driven, amongst other factors, by changes in the population size and structure, changing morbidity, technological and medical advances as well as an overall increase in national income. In respect of income, the OBR highlights that, as a “normal good”, people tend to demand more healthcare as their incomes rise. These drivers are commonly cited in the literature, including by international institutions such as the Organisation for Economic Co-operation and Development and the International Monetary Fund. It is difficult to separate their effects as they are not independent from one another.</p>

answer text <p>NHS England held a consultation on specialised gender identity services for adults (those aged 17 and above) which closed on 16 October 2017 and over 800 responses were received.</p><p> </p><p>As a consequence of the significant response NHS England had, an independent third party organisation has been instructed to provide analysis of the responses. The report will be considered by NHS England before making final decisions on future service specifications during spring of 2018.</p><p> </p><p>NHS England is committed to developing a new delivery model for patients accessing gender identity services that will offer high quality care supported by the 18 week referral to treatment standard and in line with the values of the NHS Five Year Forward View.</p> more like this

answer text <p>18 week Referral to Treatment waiting times for gender identity clinics is not routinely collected centrally.</p><p> </p><p>NHS England has confirmed that routine reporting of waiting times for specialised gender identity clinics will be implemented during 2018/19.</p><p> </p><p>NHS England consulted on gender identity services for adults in July 2017 and noted that ‘Demand for services has increased significantly in recent years, far exceeding capacity; waiting times are excessively high and are routinely in breach of NHS Constitution requirements (in many cases over 52 weeks for a first appointment)’.</p><p> </p><p>There have been no recent meetings between NHS England and my Rt. hon. answer text <p>The Government expects trusts to scrutinise the deaths of all patients who die under their management and care. The Learning from Deaths policy sets out the minimum requirements in relation to deaths due to problems in care. We would also expect trusts to continue to apply their existing processes for reviewing unnatural/unexpected deaths.</p><p> </p><p>From Quarter 3 of 2017-18, trusts will publish data each quarter on the number of deaths more likely than not to be due to problems in care (including deaths of individuals with mental illness or a learning disability).</p><p> </p><p>Certain deaths should be reported by trusts to other agents for investigation, notably the coroner, and this includes deaths thought to be unnatural or violent and every death of patients subject to detention under the Mental Health Act 1983.</p><p> </p><p>On 1 April 2017, the Healthcare Safety Investigation Branch (HSIB) became operational with the overarching aim of investigating certain serious patient safety incidents that occur during the provision of NHS services (including mental health services) with a focus on learning. Directions provide for the HSIB to set its own investigation principles for determining which incidents to investigate.</p><p> </p><p>The Health Service Safety Investigations Bill was published in draft on 14 September 2017 for pre-legislative scrutiny. The Bill will establish a new, independent Health Service Safety Investigation Body that will operate at arms’ length of Government and take forward the work of the existing HSIB.</p>
