answer text <p>The monitoring of performance against the 95% four hour accident and emergency standard, along with data regarding 12 hour waits for admission, bed occupancy and delayed transfers of care is collected and published monthly by NHS England. Data, published at both a national and a trust level, is available via the following link:</p><p><a href="https://www.england.nhs.uk/statistics/statistical-work-areas/" target="_blank">https://www.england.nhs.uk/statistics/statistical-work-areas/</a></p><p>From 2015-16, NHS Continuing Healthcare has been included in the assurance processes for clinical commissioning groups (CCGs).This helps NHS England ensure CCG compliance with the ‘National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (November 2012) (revised)’. The Government’s 2017/18 mandate to the National Health Service outlines that every hospital and its local health and social care partners must have adopted seven day discharge capabilities to help enable appropriate patient flow.</p><p> </p><p>Discharge to Assess and Trusted Assessor arrangements are both monitored by the Better Care Fund (BCF) partners. This is supported by BCF quarterly reports that are submitted by Health and Wellbeing Boards to NHS England as part of the policy framework and planning requirements for 2017-19.</p><p> </p><p>The good practice guide; Focus on improving patient flow for national priorities for acute hospitals 2017 was published by NHS Improvement in July 2017. answer text <p>The allocation of funding to local clinical commissioning groups (CCGs) is informed by the estimation of the relative health needs of local areas, based on a set of funding formulae. This formula is based on an assessment of factors such as demography, morbidity, deprivation, and the cost of providing services in different areas.</p><p> </p><p>The populations used in the funding formula for each CCG are projected forward at CCG level for each year 2016-17 to 2020-21, based on Office for National Statistics’ population projections.</p><p> </p><p>In 2016, NHS England also established a national Demand and Capacity programme to support the NHS to better understand demand and plan sufficient capacity, so that patients do not wait unnecessarily for treatment. The programme is jointly sponsored by NHS England and NHS Improvement.</p> more like this

answer text <p>Since 2014, Public Health England has an on-going contractual relationship with the Institute for Health Metrics and Evaluation (IHME) to provide data and quality assure the modelling work undertaken by IHME.</p><p> </p><p>The Global Burden of Disease study (GBD) 2013 findings were published in a series of papers on subjects such as: smoking; overweight and obesity; HIV/AIDS, malaria, and tuberculosis; disability-adjusted life years, and risk factors. The papers are available at:</p><p><a href="http://www.thelancet.com/gbd/2013" target="_blank">http://www.thelancet.com/gbd/2013</a></p><p>Starting with GBD 2015, annual updates of GBD estimates are being produced to provide policymakers, donors, and other decision-makers with the most timely and useful picture of population health. answer text <p>Funding for new primary care centres and healthcare hubs is primarily provided from two sources: the Estates and Technology Transformation Fund (ETTF) which is administered by NHS England; and additional money for schemes that has been announced from the capital Sustainability and Transformation Partnerships (STPs) budget funding.</p><p> </p><p>The ETTF has committed to spend at least £700 million of capital and revenue between 2015/16 to 2019/20 to support investment in infrastructure in general practices, increasing capacity and flexibility of premises to be fit for the future and to implement technology solutions to enable new and improved ways of working. The programme includes new premises, but the split between categories is not centrally available.</p><p> </p><p>The following schemes have been announced to receive funding from the STP capital provided in the Spring Budget and Autumn Budget and relate to the development of primary care centres and healthcare hubs. The funding is contingent on completing the established business case approvals, and the amounts will be finalised at that time.</p><p> </p><p>Spring Budget</p><p> </p><p>- Cotgrave Surgery Integrated Care Hub scheme (Nottingham and Nottinghamshire STP) to provide a new integrated health component of multi-service centre benefitting 12,000 residents of new homes developed in the area.</p><p> </p><p>- North Clacton Primary Care Hub scheme (Suffolk and North East Essex STP) to create three general practitioner practices, Care Closer to home services and a Renal Dialysis Unit.</p><p> </p><p>- Bedford Hospital Primary Care Hub scheme (Milton Keynes, Bedfordshire and Luton STP) to improve GP access for local communities and support urgent care at the hospital’s accident and emergency department.</p><p> </p><p>- Strelley Health Centre Redevelopment (Nottinghamshire STP) to accommodate future growth of existing services plus additional primary, community and social care services including a pharmacy, utilising the current site.</p><p> </p><p>Autumn Budget</p><p> </p><p>- Frimley Out of Hospital Integrated Care Hubs scheme (Frimley Health STP) to enable patients to be treated out of hospital.</p><p> </p><p>- Chiltern and Aylesbury Vale Primary Care Hub scheme (Buckinghamshire, Oxfordshire and Berkshire West STP) to allow the expansion of community teams and care provision outside of traditional hospital settings in primary care hubs.</p>

answer text <p>The Five Year Forward View made a specific commitment to do more to support people with long term conditions manage their own health. Further to this, sustainability and transformation partnerships (STPs) have a remit to consider how to improve self-care.</p><p> </p><p>In 2015 NHS England established the Realising the Value Programme. This was designed to identify evidence-based approaches that engage people in their own health and care and develop practical tools to support implementation. The programme has now published tools and guidance for local health economies, as well as an economic modelling tool to help services understand the costs and benefits.</p><p> </p><p>NHS England is also rolling out the Patient Activation Measure (PAM). The PAM is a tool which captures the extent to which people feel engaged and confident in taking care of their health and wellbeing, helping professionals to tailor support. NHS England has agreed to grant 1.8 million people with long term conditions across 27 areas access to the tool.</p><p> </p><p>Eight STP areas to take part in its new one year Building Health Partnerships programme supported by NHS England to facilitate strong engagement with the voluntary sector and local communities on actions that improve wellbeing and self-care. NHS England is also working with 15 new care models across the country to test how to deliver support for self-care, systematically and at scale. By working with the new care models, NHS England plans to offer tailored self-care support to 25,000 – 30,000 people by end of March 2018, delivered through health coaching/self-management education and social prescribing, supported by use of the PAM.</p>

answer text <p>Local commissioners have been asked to review their current local urgent care facilities against the key Urgent Treatment Centres standards and make a decision for each facility; these decisions should be based on local patient need and take into consideration current and planned future provision of urgent and primary care services.</p><p> </p><p>NHS England is working with clinical commissioning groups to develop complementary online channels to operate alongside existing services, which offer patients choice and flexibility. This includes the deployment of NHS 111 Online to all areas in England, allowing people to connect with urgent health care services through either a web-based or app-based interface.</p><p> </p><p>NHS Improvement is leading the effort to standardise and enhance the service offer, make sure services are best positioned to reduce unnecessary attendances at accident and emergency departments, and help patients understand what services are available wherever they are in the country.</p>

answer text <p>NHS England’s consultation on draft guidance for clinical commissioning groups on a range of items that should not routinely be prescribed in primary care ended on 21 October. The draft guidance seeks to ensure that the National Health Service is able to secure best value from its use of medicines and therefore focuses on areas of prescribing that have been found to be ineffective or where there are more cost effective alternatives. A full Equality Impact Assessment was published alongside the draft guidance and a further Assessment will be published when final guidance is produced.</p><p> </p><p>We would not want to pre-empt any outcome following NHS England’s analysis of the responses it has received. However, we are assured that as part of issuing the final guidance, careful consideration will be given by NHS England to the responses to the consultation, both to ensure that particular groups of people are not disproportionately affected and that principles of best practice on clinical prescribing are adhered to.</p>
