On joining the Army, all soldiers are provided with clothing designed to protect them from all climate conditions experienced in the UK and Western Europe. answer	answer is ministerial correction date of answer 19/03/2019 answer text <p>Young mothers with breast cancer should receive personalised care in line with the NHS England Long Term Plan commitments for personalised stratified follow up and personalised care, taking into account the particular needs of people in this situation.</p><p> </p><p>The NHS Cancer Programme aims to improve access to cancer Clinical Nurse Specialists (CNS) for all cancer patients when they need it, including those with secondary breast cancer. Health Education England is also working to expand the number of CNSs and develop clear CNS competencies and routes into training. NHS England is committed to ensuring that more patients have access to a CNS or other specialist from diagnosis onwards to guide them through treatment options and ensure they receive appropriate information and support. 91% of patients reported having access to a CNS in the 2017 Cancer Patient Experience Survey. The new NHS Long Term Plan commitment is that all patients, including those with secondary cancers, will have access to the right expertise and support, including a CNS or other support worker by 2021. However, they were confirmed for retirement in 2014 and updates to the system are no longer being provided.</p><p> </p><p>SNOMED CT (Systematized Nomenclature of Medicine - Clinical Terms) is the clinical terminology that has replaced Read Codes within the NHS. It is a structured clinical vocabulary for use in an electronic health record and is the standard for communication of clinical information between providers of care. SNOMED CT is an approved information standard published under section 250 of the Health and Social Care Act 2012.</p><p> </p><p>Within SNOMED CT, there are currently 87 concepts, which represent a unique clinical meaning, related to the recording of chronic pain, including chronic back pain, chronic neck pain and chronic pain control. Reporters Without Borders (RSF) reported that 80 journalists and media workers were killed in 2018. An additional 300 are currently in detention and 60 are being held hostage. We are very concerned about the high number of countries using restrictive laws to stifle freedom of expression and prevent the functioning of an independent media.</p><p> </p><p>The UK believes that access to fair and accurate information is the lifeblood of democracy. As such we are committed to the promotion of media freedom and the protection of journalists, which is why we have launched a global Media Freedom Campaign for 2019. Our aim is to shine a spotlight on media freedom, and in so doing to raise the cost to those abusing it.
